General Summary

Provides spiritual care and guidance to patients, families and staff and performs or arranges for religious services to meet patient needs.

Essential Functions:

Provides direct spiritual care to patients/families/caregivers/staff, when needed and assessed.Participates in a spiritual assessment of the patient/family/caregivers, as indicated.Facilitates the development of a plan of care to meet the identified spiritual needs in conjunction with the interdisciplinary team.Serves as liaison to community chaplains and faith community leaders, as indicated.Documents all patient/family/caregiver contacts and provides additional written, pertinent information as indicated.Participates in patient care conference and interdisciplinary team meetings.Provides consultation, education and support to the interdisciplinary team on spiritual care.Provides for funeral or memorial services or other services as requested by patient/families.Provides bereavement support.Works across region for the department when needed for coverage.

Qualifications

Minimum Education:

Bachelors Degree Required orMasters Degree for APC Certification Required

Work Experience:

2 years Relevant experience. Required

Courses and Training:

Ordained/Endorsed by recognized religious body/proper ecclesiastical authority. Upon Hire RequiredCertified (or eligible) Chaplain by Association of Professional Chaplains (APC) Upon Hire Preferred orNACC, NAJC Upon Hire PreferredGraduate of accredited seminary or school of theology Upon Hire Preferred
