Hospice Chaplain
Reports to: Assistant Manager of Hospice
Duties and Responsibilities:
+ Ability to conduct a comprehensive spiritual assessment of hospice patients and families.
+ Develops and implements an individualized care plan based on the patients physical, social, spiritual, cultural and educational needs. Involves the patient and family in the care plan.
+ Ability to provide non- denominational spiritual support and accepts the beliefs and values of the individual patient and family.
+ Documents assessments, interventions, response to interventions, goals and plan in the EMR timely.
+ Identifies barriers in meeting spiritual needs of patients and families.
+ Participates in the IDT meetings. Collaborates with team members on the spiritual and emotional needs of patients and families.
+ Documents updates in the Interdisciplinary care plan weekly.
+ Provides spiritual support and education to the interdisciplinary team as needed.
+ Participates in the annual Service of Remembrance.
+ Performs appropriate rituals and services related to end of life care as desired by patient and family.
+ Contacts patients /family’s faith community as requested.
+ Orients and precepts new employees as needed.
+ Identifies patient safety needs.
+ Other duties as assigned
Other Tasks Which May be required: Knowledge of age-appropriate behaviors, needs and interventions. The ability to work with people of diverse cultures and socioeconomic status.
Required:
+ Graduate of an accredited seminary school of theology with a Master of Divinity, Master of Theology, or equivalent degree OR a clinician experienced in end-of-life counseling with hospice experience and has been educated on non-denominational spiritual counseling or care.
Preferred:
+ A Minimum of 2 Clinical Pastoral Education Units preferred.
+ Experience with Hospice/ Palliative care or Geriatric patients
+ Ability to communicate effectively with community clergy and congregations.
