CHF Coordinator

Job Summary

The nurse navigator utilizes a focused approach in following and directing inpatients with CHF to the after discharge clinic. In the process of following and directing those inpatients the CHF navigator will educate the patient/family/caregiver regarding the disease process and the necessity of close, continual monitoring to negate the need for inpatient admission/readmission. The CHF Nurse Navigator will possess the attributes of a leader/self-starter/team player enabling them to serve as a bridge between the professional staff and the patient/family/caregiver. The CHF Nurse Navigator will possess exceptional communication skills, both written and oral that will allow them to build collaborative relationships to enable the patient/family/caregiver to better navigate the health care system. The CHF Nurse Navigator will work closely with the CHF Coordinator to ensure DNV certification is attained and maintained.

 

Responsibilities

• Assumes leadership for the CAMC CHF Program. As such, drives execution by translating strategic priorities in operational reality. Drives improvement by leading through vision and values for the CHF patient population. Demonstrates the ability to lead. Assumes the oversight for the CHF Program. Accountable for coordination of CHF Certification activities for CAMC. This includes submission of any required written evidence of standards compliance and audits to the certification body. Accountable for coordination of survey process for CHF certification. Serves as liaison with DNV and the statewide network for completion of applications for surveys, coordination of survey schedules, preparation and submission of any written evidence of compliance and coordination of any unannounced surveys for compliance. Formulate correction action plans and preventative action plans and evaluate the effectiveness of these in meeting the intent of the DNV requirements.

• Accountable for development of CHF Team goals in alignment with CAMC corporate level goals. Serves as consultant in interpretation of CHF Certification standards and strategies to meet the intent of the standards. Maintains current knowledge of standards and stays abreast of changes from the certification body. With the Medical Director, serves as Chairperson of the CHF Committee, Chairs the Evidence Base Care Team. Provides data necessary to determine opportunities, develop plans, and sustain success. Accountable for annual self-assessment of medical center to meet the requirement of the certification body. This assessment is based on data and collaboration with the CHF Team. Works with CHERI to identify researchable issues within the CHF Program. Responsible for oversight, coordination, and participating in resulting studies and encourage and facilitate those findings into practice. Delegates and supervises the interaction of all members of the CAMC Cardiology department, to assure that appropriate deadlines are met for the research studies timelines.

• Responsible for ongoing management of documents required at time of surveys. Responsible for maintaining awareness of national trends and advancements in areas of responsibility and demonstrating commitment to process of continuous quality improvement. Facilitate the timely implementation of the plan of care in collaboration with the multidisciplinary team. Monitor and evaluate, in collaboration with the Medical Director, the function of the CHF Team related to: a. Care delivery vs. outcomes b. Cost at discharge for care delivered c. Appropriate and efficient use of resources throughout the CHF continuum of care d. Comparison of center functions with like-level centers e. Coordinate and facilitate necessary changes in the CHF Team. Prepare grant proposals to seek funding support for CHF initiatives.

• Interface with attending and consulting physicians, residents, staff nurses, and other members of the multidisciplinary team to identify and address system processes, which impact the timely facilitation and delivery of care. Address these issues to provide optimal CHF care. Works in direct collaboration with the CHF Medical Director, Nurse Managers, CHF Clinic and hospital support of day-to-day operations, clinical management program planning, development, and marketing. Develop, coordinate, and provide community outreach activities for prevention, education and health screenings of participants. Includes development of education for CAMC patients and identification of Performance Improvement issues.

• Monitors clinical outcomes and system issues related to quality care delivery, developing quality filters, audits, and case reviews, identifying trends and helping to outline remedial actions while maintaining confidentiality. Using Performance Improvement data to benchmark with state and national center. Responsible for the implementation, review, and revision of CHF standards, policies, procedures, and protocols in collaboration with the CHF Medical Director. Participate in performance improvement activities in order to share data and develop methods to optimize resource utilization in order to achieve quality outcomes. Utilize tracking methods to facilitate system changes or change in practice and participate in quality management activities, including screening, focus studies, and others. Incorporates data and information to continuously improve care and practice to enhance outcomes.

• In collaboration with the interdisciplinary team, develops, maintains, and monitors CHF practice, guidelines, order sets and documentation.Make decisions affecting public relations and community interactions in collaboration with Community Affairs. Specifically the employee must be able to demonstrate competency in: 1) ability to obtain and interpret information in terms of patient needs; 2) knowledge of growth and development; and 3) understanding of the range of treatment needed by the patients.

Education 

• Associate's Degree (Bachelor's Degree preferred ) Experience: 5-7 Years experience
