Chronic Care Coordinator
Works in collaboration with the primary care providers and all members of the healthcare team. Identify patients who are eligible for chronic care management and obtain consent from patient to participate in the program. In collaboration with patient’s primary care provider create a comprehensive assessment and care plan to support disease control and health management goals, including outside resources, community support, referrals, and educational information. Provides additional support and maintains regular contact with patients between visits and provides more frequent communication with providers. Completes and documents information in Electronic Health Record (EHR). Coordinate and manage care for patients by assisting patients in navigating their care across the continuum. Education: High School Diploma or GED required. Graduate of a Registered Nursing program, Practical Nursing program, or CAAHEP or ABHES accredited medical assisting program preferred or a combination of education and experience will be considered. Certification: A current CMA or RMA credential through one of the following: American Association of Medical Assistants, American Registry of Medical Assistants, American Medical Technologists, National Center for Competency Testing, National Health Career Association, or National Association for Healthcare Professionals. BLS Certification required. Licensure: Current valid NC RN or LPN License. 2 years of primary care experience required.
