Client Monitoring Specialist Field - Lake Charles

General Working hours: M-F 8am - 5pm

Hourly rate $15.00

\n

Principal Duties and Responsibilities:

\n\n\n Prepare for visits by reviewing current LA Plans of Care and other data on participants in OPTS and LOCS Long Term Care Programs to applicants\n Comply with routine participant monitoring in-person visits \n Complete Monitoring Checklist in LOCS, review provider service logs, and time sheets during in-person visits \n Document participant changes, outcomes, and any participant issues or complaints\n Complete Elder Protective and Adult Protective forms, as needed. \n Route provider issues via LOCS to the Provider Monitoring Unit within one business day \n Reported change in status via LOCS to the appropriate Client Assessment Specialist within one business day\n Comply with complaint procedures, escalate and refer complaints to OAAS for Protective Service Services according to OAAS policy\n Daily travel to scheduled visits - must have own reliable vehicle\n Participate in ongoing Conduent and Departmental trainings\n Participate in Continuous Quality Improvement (CQI) process\n\n\n

Educational, Training, and Experience:

\n\n\n Associate’s degree in health or human services, preferred\n Minimum of two years of health-related service experience; at least one year experience working with older adults or persons with disabilities\n Knowledge of healthcare, elderly and disabled adult populations preferred\n Knowledge of Medicaid preferred\n\n\n

Specialized Knowledge and Skills:

\n\n\n Effective verbal, written, and interpersonal skills\n Excellent analytical skills\n Ability to interpret data \n Ability to work independently and autonomously\n Ability to adhere to OAAS and CSS departmental timelines\n Valid Louisiana driver’s license\n Current Louisiana automobile insurance\n Working knowledge of and experience in:\n Department of Health and Hospitals (DHH)\n Office of Aging and Adult Services\n Bureau of Health Services Financing (BHSF)\n Medicaid\n Office of Citizens with Developmental Disabilities (OCDD)\n Medicaid Home and Community Based Waivers\n Understanding of healthcare, especially in areas related to elderly and disabled adults\n
