GENERAL DESCRIPTION:
Reporting to Director of Client Services, the position supervises the peers, patient navigators, and outreach staff as well as provides services to persons diagnosed with HIV. In addition to managing a caseload, the position participates in the triage of client situations and direct training of coordination staff.
PRIMARY RESPONSIBILITES:
Provide direct triage of patient navigator, outreach, and peer clients.
Provide supervision of assigned staff. (patient navigators, outreach, and peer advocates)
Assist in training of client services staff.
Initiate referrals to agency and community services providers, as necessary, and coordinate activities in order to provide an integrated system of service delivery.
Document services delivery and maintain client records and confidentially in accordance with AOC polices & procedures
Report client services statistics; attend and participate in the agency related meetings, retreats, work sessions, and events as requested; and perform other related duties as assigned by the Director of Client Services.
QUALIFICATIONS:
Professional certification or license; working knowledge of HIV/AIDS; cultural knowledge and sensitivity; excellent written and verbal communication skill; knowledge of community resources; ability to handle crisis situations; word processing skills; act as a liaison between AOC and the community to facilitate positive relationships at all time; previous supervisory experience; ability to complete assigned task in a timely manner; demonstrate a non-judgmental attitude toward clients, staff and volunteers; ability to discuss sex/sexuality issues in appropriate manner with clients; ability to adapt quickly to change; valid Texas driver’s license;
