**Primary Function/General Purpose of Position**
The Clinical Care Coordinator collaborates with the assigned care team to coordinate the process of patient care from the point of admission to the hospital, through discharge and/or hand-off to a post-acute care provider.
HIV Program: The Ryan White Clinical Care Coordinator collaborates with the Ryan White care management team to coordinate the process of outpatient care to ensure that care being rendered is indicated as the HIV Standard of Care, resources utilized are appropriate, and services provided are in the appropriate outpatient setting.
**Essential Job Functions**
+ Actively participates in daily meeting with hospitalist team to ensure plan of care for patients is communicated and acted upon. Collaborates with hospital care team to ensure that all current labs, test results, consults, orders, etc. are carried out in a timely manner.
+ Meets with patient/family to determine barriers to discharge and set expectations for hospital stay, coordinates with case management in the resolution of any post-discharge homecoming issues
+ Communicates pertinent information regarding patient’s hospital stay with patient’s primary care provider as needed; ensures follow-up appointment is made with primary care provider.
+ Under supervision of Program Medical Director and in collaboration with hospital’s utilization management, evaluates clinical care
+ Meets with patient and families on admission to educate on role of hospitalist; educates and promotes hospitalists program among hospital staff
+ Performs other job duties and responsibilities as required.
HIV Program
+ Evaluates and monitors services provided directly or indirectly utilizing grant funds from the Ryan White Modernization and Treatment Act to determine patient clinical outcomes and to ensure that services meet the needs of persons living with HIV and adhere to the Public Health Service Standards of Care
+ Assists the Program Manager and Program Analyst to ensure quality improvement in all aspects of the clinical service delivery and identification of new service development
+ Assist Program Manager with development of Policy and Procedures for the clinical team
+ Serves as the Supervising Clinical Case Manager for the Program. Is the intake coordinator and assigns patients to the appropriate Case Manager. Carries a patient Caseload and performs assessments and reassessments of continuing care needs
+ Provides oversight and supervision of the Clinical team to ensure core clinical services are provided to all enrollees as mandated by the Ryan White Care Act
+ Provides crisis intervention, information, and referral services to social needs
+ Identify and respond appropriately to ethical issues in patient care and act as resource to others in addressing ethical dilemmas in patient care
+ Performs other job duties and responsibilities as required.

**Licensing/Certification**
Currently licensed as a Registered Nurse in the state of South Carolina or holds a current compact/multi-state license as a Registered Nurse in a recognized NCSBN Compact State and is not a permanent resident of SC. Must have a current American Heart Association BLS for Healthcare Provider Card
**Education**
Registered Nurse: BSN (preferred)
**Work Experience**
Minimum of 3 years as an RN in an Acute Care Hospital. Case Management Skills preferred.

**Working Conditions/Physical Requirements**
Frequent sitting, walking, standing. Intermittent stooping, bending. May require lifting or moving items up to 25 lbs. Frequent use of finger/hand dexterity. Constant talking or hearing. Corrected hearing and vision to normal range. Possible exposure to blood, body fluids or tissue. Possible exposure to infectious materials, communicable diseases and/or other conditions common to a healthcare environment. Requires work under stressful conditions, deadlines, and/or irregular hours. Ability to read and comprehend. Travel may be required.
