Summary

The Clinical Care Leader (CCL), Assisted Living is responsible for utilizing the nursing process (assessment, diagnosis, outcomes/planning, implementation, and evaluation) to provide individualized nursing care in the assisted living setting.

Job Description

Collaborates with other inter-professional colleagues, to plan, implement, and evaluate care. Documents patient/resident history, symptoms, medications, and care provided in compliance with applicable standards/regulations. Utilizes technology to enhance quality patient/resident care, to collaborate with inter-professional team members, effectively delegate to and supervise unlicensed personnel, assume overall nursing accountability for a patient/resident assignment, and serves in leadership roles on department/organizational task forces and committees. Demonstrates competency and practices within the full scope of nursing expertise/knowledge and utilizes appropriate age and population specific standards as designated in the specific care area or unit.

Utilizes communication skills to collaborate, negotiate, and resolve conflicts, situations, and/or issues with peers, inter-professional colleagues, patients/residents, and families. Must have strong clinical reasoning and critical thinking skills to analyze data and appropriately respond to changing situations. Performs coordination of care, patient/resident assessment, patient/resident education, triage, and various other nursing interventions.

Position will be expected to be on a rotating on-call schedule. Must be able to provide daily management to a team of resident assistant medication aides.

Functions within the scopes and standards of nursing practice as outlined in the Nurse Practice Act and Administrative Rules in state of practice and licensure. The Sanford Professional Nursing Practice recognizes the Scope and Standards of Practice and the Code of Ethics for Nurses with Interpretive Statements as published by the American Nurses Association as the foundation of nursing care delivery and professional conduct.

Qualifications

Bachelor's degree in Nursing strongly preferred. Graduate from an accredited nursing program, including, but not limited to, those accredited by the Commission on Collegiate Nursing Education (CCNE), Accreditation Commission for Education in Nursing (ACEN), or National League for Nursing Commission for Nursing Education Accreditation (NLN CNEA).

Minimum of two years’ clinical experience preferred.

Currently holds an unencumbered RN license with the State Board of Nursing where the practice of nursing is occurring and/or possess multistate licensure if in a Nurse Licensure Compact (NLC) state. Obtains and subsequently maintains required department specific competencies and certifications. Depending on location, valid driver's license may be required. Basic Life Support (BLS) required. Resident Assessment Instrument (RAI) process experience preferred, but not required.
