Clinical Case Manager, University Counseling Center

The Clinical Case Manager (CCM) in the Vanderbilt University Counseling Center (UCC) works closely with the Associate Director for Clinical Operations and UCC Counseling/Psychiatry teams in providing case management services to students, and supporting UCC staff through planning, implementing, and coordinating resources based on individual student and staff needs. The goal of the CCM is to promote continuity of care for students receiving services from Vanderbilt University's Student Care Network as well as community resources including Vanderbilt University Medical Center (VUMC); assist UCC staff with the hospitalization process, included but not limited to: discharge planning, follow-ups with hospital staff (social workers, treatment teams), and coordination of other services; establishes, maintains and coordinates relationships with campus and external resources that would assist with the students’ needs; consults with other staff to discuss resources; consults with students to determine their needs and best course of treatment; provides transitional support to referring therapist and clients in need of referral to community services; acts as liaison and coordinates between the Student Care Coordination (SCC), UCC and community referral sources, including hospitals, private practitioners, clinics, psychiatry, and other mental health agencies while maintaining a shared database of community referral resources in partnership with the SCC. 

Duties will include, but are not limited to: tracking of no-shows/cancellations and high-risk clients; scheduling referrals; obtaining authorization to release and obtain client information and/or medical/psychotherapy records; assisting and tracking staff caseloads, coordinating hospitalizations and/or post-hospitalization care; providing education, emotional support, preventative care and health promotion to students directly; tracking interventions and outcomes of UCC services; assisting with ADHD assessment team, managing referrals within/to the center and with community providers; engaging in Drop In Consultation hours, assisting with academic calendar demands requiring coordination of care with partnership with Student Care Coordination as well as informing UCC staff of upcoming deadlines throughout semester; and providing triage, assessment, follow-up, and crisis intervention as needed. This position will also perform occasional community outreach, consultation, and liaison activities. 

Multicultural awareness, and specific awareness of racial and identity-related health equity issues, and responsiveness are important components of all clinical and community outreach oriented work.

About Student Care Network:
The Student Care Network (SCN) is comprised of the Center for Student Wellbeing, Student Care Coordination, the University Counseling Center, and the Student Health Center. The SCN is a holistic network of services and resources pertaining to health and wellness available to all Vanderbilt University students who have paid the Student Health Fee. As such, this position is expected to work collaboratively with others within the SCN to create integrated programs and services to provide timely support to our students.

Key Functions and Expected Performance: 

Provide clinical case management services to students: 

Track no-shows/cancellations and high-risk clients Schedule internal and external referrals requested by UCC clinicians, including obtaining authorization to release and obtain client information and/or medical/psychotherapy records. Develop effective working relationships with the Vanderbilt community, including Student Care Network entities, academic deans, faculty, residential staff, Dean of Students departments, Vanderbilt University Police Department, and other relevant campus partners, to ensure a collaborative support network on campus to respond to student needs and optimize support and safety for Vanderbilt students. Assist other Student Affairs offices with scheduling UCC-specific assessments as requested Monitor and update UCC Clinicians of midterm deficiencies, academic dismissals or probations of Students on their caseloads Participate in weekly High Acuity meetings with other Student Affairs offices to provide clinical updates and insight to ensure best student outcomes during weekends or University breaks. Provide outreach and educational programming to campus community to support campus and individual mental health, wellness, and skill development. Maintain appropriate and timely documentation for recordkeeping and communication with other service providers based on ethical and legal standards and requirements to enhance care for students. Engagement in urgent care support and follow-ups. Including responding to after hour reports the next business day.

Provide specialized crisis intervention and follow-up services:

Coordinate hospitalizations and/or post-hospitalization care in collaboration with the SCC, providing education, emotional support, preventative care and health promotion, tracking interventions and outcomes, and providing triage, assessment, and crisis intervention as needed Respond to mental health emergencies in a culturally responsive manner to support students in a framework and manner that acknowledges their unique and intersecting personal and cultural identities Identify appropriate interventions for students in crisis, working effectively with the staff of the UCC to access appropriate resources, and engaging campus partners effectively to develop an effective plan for supporting students. Engage in the urgent care team for working with students in crisis. Follow up with Students 24-48 hours after seen on Urgent Care. Engagement and assisting Students with MLOA recommendations. Communicate with campus partners supporting students in crisis and develop intervention strategies to ensure student safety and support. Collaborate with the Vanderbilt University Medical Center Emergency Department, Vanderbilt Psychiatric Hospital, the Partial Hospital Program at Vanderbilt University Medical Center, and other community providers, as appropriate, to ensure positive student outcomes, a smooth transition to outside care, and/or a seamless discharge/transition back to campus for students.

Training and development: 

Participate in training activities including case conference, small group supervision, and seminars/workshops to support trainee development and progress within the UCC’s accredited training program.

Perform other administrative functions: 

Serve on divisional committees and working groups to enrich the experience and resources available to staff across the division. Participate in division-wide events (Move-In Day, Rites of Spring, Diverse ‘Dores Day, MLK Day, etc.), as needed, to support colleagues and ensure these events run smoothly. Learn and comply with financial policies and procedures related to travel, procurement, and expense reporting in order to be a good steward of university funds. Engage in professional development, including presentations at pertinent conferences and workshops, to expand upon and bring back skills and knowledge to the greater UCC team. Perform other duties necessary to enhance the student experience and/or facilitate the success of the University, division, or department.

 

Supervisory Relationships: 

This position does not have any supervisory responsibility; the position reports administratively and functionally to a UCC Associate Director for Clinical Operations. 

 

Education and Certifications: 

A Master’s degree from an accredited institution of higher education in counseling, social work or other mental health-related field is required. Eligibility for licensure (LMFT, LPC, LCSW, or similar) in Tennessee within one year of employment is required. Current licensure in TN or another state is preferred.

 

Experience and Skills: 

3+ years of professional experience in the counseling space or related field is required. Demonstrated ability to effectively interact with patients/ clients from diverse backgrounds is preferred. Effective interpersonal skills to form and maintain meaningful relationships with students, colleagues, and all stakeholders preferred. Demonstrated skill in written and oral communication, including public speaking and group meeting facilitation is preferred. Ability to function in an acute care model for assessment and management of clinical emergencies is preferred.
