Clinical Case Manager
Job Description The Foster Care Clinical Case Manager is responsible for the clinical care and management of client caseloads, specifically for clients at high risk of psychiatric hospitalization or interventions. Key duties include: + Weekly therapy, monitoring treatment and service planning processes, managing information flow among persons involved with clients, and collaborating with collateral partners, staff, and foster parents. + The primary role involves providing intensive and clinical visits with clients according to clinical contracts, conducting regular mental health monitoring, and ensuring timely crisis response. Qualifications + Master’s Degree and Licensure eligible + Ability to read, analyze and interpret client information, reports, clinical documents, education plans and the like. + Strong interpersonal skills and ability to develop strong working relationships. + Cultural competency across diverse populations. + Experience working with at risk populations in clinical therapeutic settings. + Valid MA drivers license with ability to travel if needed.
