Clinical Case Manager

The Clinical Case Manager - Adoption Support and Preservation (ASAP) program provides healing-centered, family systems based therapeutic services to adopted children and their families with a goal to maintain placement and improve the overall well-being and attachment of the child in the home environment. As a Clinical Case Manager, you will assess the behavioral health of participants in community-based and/or grant-funded programs, as needed as well as provide clinical assessment, as required by program scope, and provides or recommends the appropriate therapeutic intervention.

Candidate qualifications:

Master’s Degree in Social Work or a related field preferred or Bachelor's Degree in Social Work or a related field with at least two years of relevant experience required. A valid driver’s license, insurance and a reliable vehicle required.

Compensation: Salary Range $50,336-$55,360k; offers are commensurate with experience and bilingual candidates may receive additional financial compensation. The salary range provided represents our current estimate for this role at the time of posting and is subject to change. The final salary will be determined based on a range of factors, including but not limited to, a candidate's relevant experience, education, qualifications, skills, certifications, and alignment with organizational needs. Location: (Hybrid) This role includes a combination of community-based, in-office, and remote work. It also requires travel throughout the nine Rockford counties, with in-person attendance at the Rockford home office for supervisions, team meetings and agency events.
