Clinical Case Manager

​With the supervision of the Program Manager, this position works collaboratively with the IHH interdisciplinary team to promote, enroll, and monitor the whole health to eligible clients.

Promotes access to behavioral health and physical health promotion services. Participates in care coordination activities and case discussions for IHH clients. Provide marketing and referrals for enrollment in the IHH program and whole health services. Regularly monitors and ensures understanding of each IHH member’s concerns and the type of whole health resources needed. Create on-going health promotion opportunities for all IHH clients. Supports on-going collaboration among Behavioral Health and Primary Health providers and staff. Consistently applies tools such as Wellness and Recovery Action Plan (WRAP) programs, Care Coordination Plans and symptom response plans. Meet with patients at Medical Appointments and/or other Community Locations. Provide support to other IHH staff as needed. Other duties as assigned.

Qualifications 

Current Iowa license as a Registered NurseExperience working on a multi-disciplinary teamAbility to effectively and professionally manage difficult patient issuesValid Iowa Driver's LicenseAbility to meet clients in their homes or in public places with the ability to drive clients safely, and a willingness to transport clients in own vehicle as needed.
