Coordinate, schedule, and track all one-time medical consultations. + Meeting Facilitation and Coordination: Facilitate clinical services meetings + Client Management: Communicate directly with clients and stakeholders to complete the consulting process. Maintain accurate records of consults within the secure database management system. + Training and Development: Assist in organizing and providing clinical training for healthcare providers and general training regarding clinical services for other professionals. Contribute to the development of training content. + Quality Improvement and Outreach: Assist in designing and administering quality improvement programs. Help develop content for distribution and outreach efforts. Develop a tracking mechanism for services provided, including surveys. CARE Network: + Coordinate and manage details, including ongoing education accreditation, and logistics of in-person and web-based learning opportunities, and provide day-of support and management of events and sessions. + For 1 day of annual in-person provider training, 2-day new provider in-person training, and monthly web-based 1-hour learning sessions. + Provide technical support to medical and behavioral health providers and maintain a record of their completion of CARE Network requirements. + For completion of application, scope of work, registration, onboarding, annual updates (includes licensure verification), ongoing education, and submission of case data using an online database. + Accurately and timely maintain data in program databases. + Arrange and coordinate meetings with key stakeholders. + Update and maintain details of web-based learning opportunities in the CHCO learning management system. + Provide general support to CARE Network and program leadership as needed. A sub-program of KIDS, KIDS PLUS, identifies kin support for youth whose cases are highly complex and who have remained in the child welfare system without achieving permanency.The primary role of this position is to provide comprehensive clinical support and coordination for the Kempe Center (including CPT and KIDS) and CARE Network designated providers.
