Are you living your life's work? **Summary:** + Penn Medicine's Division of Hematology/Oncology is one of the largest hematology/oncology groups in the country. We are recognized nationally and internationally for our excellence in patient care, research and training. Our physicians, advanced practice providers, and nurses care for patients with all types of cancer and benign blood-related disorders, from the most common to the most complex, seeing more than 175,000 outpatient visits annually. + The APP Clinical Coordinator will be responsible for overseeing the day-to-day clinical operations in collaboration with Practice Leadership. The Clinical Coordinator will communicate new processes, assist with coverage plans for unexpected needs, and act as the subject matter expert for clinical questions and EHR optimization. The Clinical Coordinator will assist with hiring, onboarding, and orientation of new clinical staff, complete clinical competencies and evaluations. + Typical scope for this role is: + •<10 = # of staff directly and indirectly managed + •<15,000 = # of budgeted annual visits + •Complexity Factors (may involve a few of the following factors of practice complexity): multiple practice locations, high practice/facility square footage, coordination of care across several departments/entities, academic involvement (research initiatives, resident program), special clinical programs, multiple clinical specialties, complex regulatory requirements, management of external partnerships/collaborations. + in addition to the role of Clinical Coordinator, the APP Clinical Coordinator will be an active APP provider within the Division. + The Hematology-Oncology Advanced Practice Provider (APP) (Nurse Practitioner or Physician Assistant) will function as part of a dynamic team in providing care to cancer patients within the Hematology-Oncology Division at the Abramson Cancer Center. The APP will provide care to patients both independently and in collaboration with physicians within the division, coordinate multi-specialty care, and provide patient/family education and support throughout a cancer diagnosis. + The Hematology-Oncology APP will work closely with physicians, nursing staff and other members of the cancer center care team; the APP will perform histories, physical exams, and procedures in accordance with credentialing, and provide complex oncology care and patient/family education. The APP will also participate in research as appropriate, ensure regulatory compliance, document care according to standards, and prescribe therapeutic interventions in accordance with the collaborative/supervisory agreement and state regulations. + The Advanced Practice Provider is responsible to promote organizational values of patient and visitor satisfaction by demonstrating courtesy and respect for all their families, visitors, and other employees. Cares for patients in all age groups from young adults through geriatrics. Demonstrates competence in the assessment, treatment, and care of patients served. Oncology experience strongly preferred. **Clinical Coordinator Responsibilities:** + Supports Practice growth and success by allocating and using resources appropriately. + Ensures and maintains a safe working environment for patients, families, and staff. + Strives to understand and value differences in others' race, gender, nationality, and age- modifies interaction as needed. + Establishes/updates clearly defined work processes and work practices. + Maintains a clinical practice and performs additional clinical duties to maintain workflows and adequate staffing levels. + Assists Practice and/or Clinical Manager with team and individual performance management in alignment with Penn Medicine's vision of service excellence. + Manages pay practices including accurate timecard review and approval + reviews and manages all overtime utilization. + Recognizes and rewards individuals and teams for a job well done. + Assists with recruitment and onboarding of new staff. + Regularly meets with clinical staff to improve communication and build productive relationships. + Actively identifies learning needs for individual and team development. + Assists with the development of clinical staff in collaboration with entity and system professional development resources. + Monitors and ensures staff compliance with regulatory and mandatory training. + Assists the practice and/or clinical managers in maintaining regulatory compliance (JACHO, CMS, DOH, OSHA). + Maintains a focus on Quality and Safety priorities including but not limited to applications of a quality framework (i.e., FOCUS-PDSA) and High Reliability Organization (HRO) principles. + Supports excellence in patient care and service recovery initiatives + assists with safety event review and follow-up. + Assists with the implementation of change management strategies within the practice in alignment with department, entity, and Penn Medicine objectives. **APP Responsibilities:** + Provides direct clinical care for patient population according to established protocols and utilizes evidence-based practice standards in collaboration with team. + Maintains accurate and confidential medical records. Documents all medical evaluations, diagnoses, procedures, treatments, outcomes, education, referrals, and consultations consistent with organizational standards in the electronic health record (EHR). + Identifies self properly to the patient and family and clarifies the role of the nurse practitioner or physician assistant if necessary. + The APP should function within their scope and refer to the collaborating/supervising physician when outside of scope, and/or directed by the physician or when requested by the patient and/or family. Performs interventions specific to assigned patient population and collaborative/written agreement. + Participates in the performance improvement processes to meet quality of care and service standards. + Demonstrates awareness of professional responsibility as role model to peers and members of the health care team. + Perform approved therapeutic or diagnostic procedures based upon patient’s clinical status. + Addresses patient and family concerns and creates a therapeutic relationship to provide support. + Provide patient education, counseling and instruction regarding patient diagnosis, medical treatments, medications, and preventative care. + Participate in and support accreditation, compliance and regulatory activities of the organization. + Precept students and other health care learners. + Identifies patient/family needs for community resources and refers appropriately. **Education or Equivalent Experience:** + Master of Science (Required) + CRNP: Master's Degree or Doctor of Nursing Practice (DNP) in Nursing required and > 2.5 years of Hem-Onc APP experience. + PA-C: Master's Degree requires and > 2.5 years of Hem-Onc APP experience. + The APP must be reviewed by the Credentials Committee and approved by the Medical Board. National Board Certification required. **Credentials:** + Advanced Cardiac Life Support (Required) + ACLS (For providers in the acute care setting) required + Basic Cardiac Life Support (Required) + BLS/CPR, as a healthcare provider as per the American Heart Association (For providers in the ambulatory setting) required **Additional Information:** All CRNPs and PA-Cs function under the guidelines as designed by the University of Pennsylvania Medical Center medical staff guidelines and are credentialed through that body. The CRNP will be held accountable through a formal job description, delineation of privileges, and through their agreement with their collaborating physician and a Collaborative Practice Agreement. The PA-C will be held accountable through a formal job description, delineation of privileges, and through their agreement with their supervising physician and a Written Agreement All clinical work hours and associated requirements will be defined and managed under the direction of the collaborating physician/division designee, and within the Human Resource guidelines.
