Support staff in coordinating and managing care for Endeavor patients across post-acute and ambulatory settingsEnsure the Delivery of High-Quality Care Management ServicesSupport Staff Development and Professional GrowthProvide Leadership to Ensure Operational EffectivenessCollaborate with physicians, clinical leaders, and interdisciplinary teams to ensure seamless care transitions and continuity for patientsWhat you will need:
Education: Bachelor's degree in a clinical discipline required, Masters Degree preferredCertification:Clinical certification, such as case management certification, is preferredBasic Life Support for Healthcare providers (AHA) or CPR/AED for the Professional Rescuer (American Red Cross) preferredExperience: Minimum 3 years of quality management/care coordination/health care experienceExperience with ambulator services working with high-risk patients preferredStrong background with care management, clinical social work beneficialExperience working with home health and/or hospice beneficialUnique or Preferred Skills: Strategic thinker with experience in a value-based care management environment; Care Management/Utilization Review experience with CMSA or ACMA certification preferred:
