Are you living your life's work? **Summary:** + The Clinical Coordinator will be responsible for day-to-day clinical operations of the Penn Partners in Care Ambulatory Pharmacy team, while maintaining 40% clinical responsibilities. The incumbent provides patient and population care management; maintains a clinical caseload of patients by collaborating with practices to provide clinical support to "high-risk" patient populations for multiple practices in all age groups ranging from young adulthood through geriatrics. Patient care responsibilities: comprehensive medication management and medication review for high-risk patients, interdisciplinary referrals for individualized medication and/or disease management and provider group-based medication management, both transitional care management and longitudinal care management services. + The Clinical Coordinator must be an effective communicator and be able to integrate themselves within the healthcare team, assist team with day-today clinical issues, appropriately assess and escalate any pertinent issues/questions to Penn Partners in Care Management Team (Associate Director of Ambulatory Pharmacy and Director Penn Partners in Care), improve upon existing clinical infrastructure and work toward any Penn Partners in Care Pharmacy Program expansion efforts. This includes being the primary liaison for EMR optimization to streamline operations and leverage the EMR to quantify clinical outcomes. In addition, the Clinical Coordinator will also partner with quality and the population health team to improve on pharmacist value-based care initiatives. + The Clinical Coordinator will adhere to Penn Partners in Care and pharmacy policies and procedures and strive to achieve organizational goals and objectives. This role will include face to face (travel required), telephone or telehealth encounters as appropriate to staff member and/or patient needs. (Central Fill Specialty Pharmacy - Interventional Support Center - 3250 South 76th Street) Responsibilities: + Collaborates with the multidisciplinary team to support clinical outcome metrics. + Responsible for precepting pharmacy students and pharmacy residents completing ambulatory care rotations. + Works with interdisciplinary departments to drive pharmacy quality and value-based care. + Support the creation of clinical effectiveness targets, improvement plans and reporting systems to ensure the delivery of high quality, effective and efficient patient care. + Partner with quality and the population health team to improve pharmacist value-based care initiatives. + Assists with the creation and expansion of Pharmacist Collaborative Drug Therapy Management agreements across ambulatory clinics. + Primary liaison for EMR optimization to streamline operations and leverage the EMR to assess productivity and quantify clinical outcomes. + Conduct special projects assigned by the Associate Director Ambulatory Pharmacy Services and Director of Penn Partners in Care to expand or enhance pharmacy services in partnership with ambulatory clinics. + Reviews patient profiles for appropriate drug therapy, contacting Providers to optimize therapy. + Evaluates patient’s age, height, weight, pregnancy and lactation status, renal and liver function, therapeutic serum medication concentrations, to determine appropriate drug dosage regimen. + Documents interventions, cost avoidance initiatives or drug information inquiries appropriately. + Completes assignments in a timely fashion, including prompt response to provider/care management referrals. + Provides comprehensive medication management and medication reconciliation for patients as requested by the care management or prescribing staff or if indicated by clinical judgment. + Assists in transitional care management between the acute and ambulatory care setting. + Assists with longitudinal care management and chronic disease state management: including designing, implementing and coordinating medication care plans for patients on an on-going basis. + Monitors medication therapy to evaluate appropriateness of use, dose, dosage form, regimen, route, therapeutic duplication, and drug interactions. + Develops, implements and coordinates patient drug therapy, educational programs for department staff and is a pharmacotherapy specialty resource for the providers, care management and practice staff.Accountabilities. + Provides information to the medical, pharmacy and nursing staff to ensure optimal drug usage, provides recommendations to improve therapy, advises physicians on appropriate drug therapy. + Ensure compliance with all state, Joint Commission (Joint Commission for the Accreditation of Health Care Organizations) and OSHA (Occupational, Safety, and Health Administration) regulations in terms of medication management both within the pharmacy and assigned patient care unit. **Credentials:** + Pharmacist in the state of PA(Required) + Licensed Pharmacist in the state of NJ (obtained within 90 days of hire) + Completion of an ASHP- accredited PGY1 Pharmacy Residency and Board Certification by Board of Pharmacy Specialists OR Completion of an ASHP- accredited PGY2 Pharmacy Residency **Education or Equivalent Experience:** + Bachelor of Arts or Science from an accredited College of Pharmacy (Required) + 3+ years related experience (Required) We believe that the best care for our patients starts with the best care for our employees.
