Join Loretto Health and Rehabilitation as a Full Time Clinical Coordinator, RN! 

 

Under the guidance of the Unit Nurse Manager, The Clinical Coordinator RN is responsible for coordinating the clinical practice and care to residents in a Long Term Care and Short Term Rehab setting. This role is a liaison between residents, families, medical providers and Loretto employees. Conducts patient assessments, implements and evaluates the effectiveness of nursing care delivered to the residents.

Shift: Full Time Days 7:30am – 4:00pm or 8:00am – 4:30pm 

Monday - Friday (No Weekends)

Pay: $81,078.40 - $91,062.40 Yearly Salary

Responsibilities

Responsibilities include:

Oversee the delivery of Quality Patient CareCompliance with NYSDOH and CMS regulations and facility policies and protocolsOrients, trains and mentors new employeesProvides health training to patients and their familiesAssists in gathering and analyzing data for QAPIActs as primary liaison with medical providers Qualifications 1+ year experience in nursing preferredCurrent Registered Nurse NYS licenseEstablished patient assessment skillsKnowledge of best-practice standardsSkilled to respond to emergent situationsAbility to analyze problems, plan resolution strategies, implement solutions and evaluate outcomesExceptional interpersonal and customer service skillsProficiency in speaking, listening, reading and writing the English Language is required
