Our Syracuse location is comprised of 2 buildings; the Cunningham building, a 13-floor, 487-bed long-term care skilled nursing facility, and Fahey Rehab, a 96-bed short-term residential rehabilitation program - the largest in Central New York. Our Fahey building offers specialized services including Palliative Care and Restorative Care. Under the guidance of the Unit Nurse Manager, The Clinical Coordinator is responsible for coordinating the clinical practice and care to residents in a Long Term Care and Short Term Rehab setting. This role is a liaison between residents, families, medical providers and Loretto employees. Conducts patient assessments, implements and evaluates the effectiveness of nursing care delivered to the residents.
ResponsibilitiesResponsibilities include:
Oversee the delivery of Quality Patient CareCompliance with NYSDOH and CMS regulations and facility policies and protocolsOrients, trains and mentors new employeesProvides health training to patients and their familiesAssists in gathering and analyzing data for QAPIActs as primary liaison with medical providers Qualifications 1+ year experience in nursing preferredCurrent RN NYS licenseEstablished patient assessment skillsKnowledge of best-practice standardsSkilled to respond to emergent situationsAbility to analyze problems, plan resolution strategies, implement solutions and evaluate outcomesExceptional interpersonal and customer service skillsProficiency in speaking, listening, reading and writing the English Language is requiredINDNS
