Position Summary:
Responsible for individualized client services provided in a community/residential setting. Duties include assessing, planning, implementing, monitoring, and evaluating client behaviors and outcomes while ensuring appropriate level/type of services are provided.
Position Description:
With close supervision; conducts interviews and administers assessments and screening instruments Maintain caseload and productivity by ensuring regular contact with clients to monitor and assess progressWrite service plans, review progress and determine updates needed in collaboration with clientUtilize core correctional practices while assisting clients in effective community re-integrationDeliver on-going services to clients which may include: crisis intervention, advocacy, medication monitoring, transportation assistance, financial budgeting, housing assistance, and referral and linkage to community resources as neededEnsure compliance with applicable standards and agency policiesDocument all clinical activities to meet specific program, funder and regulatory criteria timely and with accuracyParticipate in team meetings and assist in making recommendations for client status changesLead educational groups and/or facilitate group curriculumMay require transportation of clients Adhere to professional standards as outlined by protocols, rules and regulationsOther duties as assignedRequired Knowledge, Skills and Abilities:
Ability to learn case management principles and core correctional practicesExcellent organizational and time management skillsEffective verbal and written communication skillsDemonstrate strong ethical and professional behavior. Ability to relate to and interact with a wide variety of people in an ethical, culturally sensitive and respectful mannerStrong problem solving skills and ability to multi-task while prioritizingAbility to utilize technology including electronic service documentation systemJob Requirements:
Bachelor’s Degree in Behavioral Science, Criminal Justice or related field or; equivalent work experience as allowed/required by regulatory bodiesMay require CDCA or other specialty certificationValid driver license and auto insuranceEssential Functions/Physical Demands:
Positional: Driving in accordance with job duties assigned. Frequent standing and walking. Frequent sitting.Gross Mobility: Rare climbing, or crawling. Infrequent balancing, stooping, kneeling, or crouching. Frequent to constant reaching and handling.Sensory: Rare tasting/smelling. Infrequent use of color vision. Occasional use of far visual acuity, depth perception, and field of vision. Frequent use of near and midrange visual acuity, and visual accommodation.
