Who Do We Serve at this Program:  Telecare In-Home Outreach Team (IHOT) is a centralized program offering mobile teams to provide in-home outreach to adults with serious mental illness (SMI) who are reluctant or resistant to receiving mental health services. IHOT offers services throughout San Diego County.

The IHOT program also serves as the gatekeeper program through which referrals to the Telecare Assisted Outpatient Treatment (AOT) program are made. The AOT program is part of a continuum of supportive services offered to individuals with serious mental illness who are opposed to engaging in treatment services and who meet the nine criteria of Laura’s Law. 

Shifts Available:

Salaried position, full time, Monday - Friday, 8:00am - 5:00pm

If you are a LCSW LMFT or LPCC: Realistic Wage Range $86,000.00 - $103,382.28 based of years of relevant experience

Telecare applies geographic differentials to its pay ranges.  The pay range assigned to this role will be based on the geographic location from which the role is performed.  Starting pay is commensurate with relevant experience above the minimum requirements.

What You Bring to the Table (Must Have)

• Must have a valid LCSW LMFT or LPCC with CA BBS.

• Prefer that you have clinical management or leadership experience. Willingness to be a leader and manager: spearhead staff development, review documentation notes, check electronic timecard, oversee mileage reimbursement, run morning meeting, partner with hospitals/county/other entities for referral and discharges, oversee compliance trainings for team, client consultations, train staff of clinical expectations, interview candidates, and more. 

• Valid and current driver’s license, and personal vehicle insurance with your name listed as a driver.

• Willingness to use your personal vehicle to drive clients to appointments and groups, etc.

• Willingness to use the company vehicle to drive clients to appointments and groups, etc.

• Willingness to use your personal vehicle to attend meetings, etc.

What You Will Do

• Assist your program leadership with implementing and overseeing the responsibilities for clinical services and standards of care for our clients, and administrative needs of the multidisciplinary team.

• Work closely with your program leadership to continually communicate with community partners and all local systems of care that are in service to our clients.    

• Oversees documentation by clinical staff and work closely with your program leadership to assist with audits of Clinical Charts to ensure adherence to State and County Requirements.

• Assists with New Hire Orientation and ongoing training and mentorship for all clinical staff.

• Assists with recruitment, screening, hiring, onboarding, performance evaluations and terminations of all clinical staff
