ResponsibilitiesThe Clinical Dietitian will plan, develop, implement and monitor the nutrition plan of care for each resident. Responsibilities include timely nutrition assessments, care planning, and MDS completion. The nutrition plan of care is accurately and clearly communicated to the interdisciplinary team, resident and families via verbal discussions and written communication. Nutrition programs and systems are routinely monitored as part of the performance improvement program.
