At FutureCare our Clinical Dietitians provide medical nutrition therapy, nutrition assessment, diet modification, education, and intervention for our resident population.  The Clinical Dietitian follows strict guidelines as outlined with FutureCares diet manual and the clinical nutrition policies & procedures manual.

Responsibilities Plan, develop, implement and monitor the nutrition plan of care for each residentProvide timely nutrition assessments, care planning, and complet Minimum Data Set (MDS) assessments as requiredhe nutrition plan of care is accurately and clearly communicated to the interdisciplinary team, resident and families via verbal discussions  and written communication.Develop nutrition programs and systems are routinely monitored as part of the performance improvement programDetermine patients at nutrition risk via nutrition screeningProvide nutrition education to patients/familiesProvide nutrition counseling and education to patientsAssure proper nutritional care for patients by conducting nutrition assessmentProvide medical nutrition therapy (nutrition assessment, nutrition screening, counseling, education, modified diet formulation, nutrition intervention) for in-patients and out-patientsProvide advanced medical nutrition therapy (nutrition assessment, nutrition screening, counseling, education, modified diet formulation, nutrition intervention, nutrition support) for in-patientsProvide medical nutrition therapy (nutrition assessment, nutrition screening, counseling, education, modified diet formulation, nutrition intervention) for in-patients and out-patientsProvide nutrition services and medical nutrition therapy integrated with the patient's medical needsDevelop nutrition education materials/tools for patientsDevelop and implement medical nutrition care plansPerform comprehensive nutrition assessments and documents nutrition interventions in patient medical recordsAssess patients for nutritional statusProviding nutrition assessments and care planning for patientsEvaluate nutritional needs and develop a nutrition care planProviding medical nutrition therapy (nutrition assessment, nutrition support, patient intervention and education) to meet the nutrition care needsParticipate as a member of the Food Service Management team directly managing patient feedingProvide recommendations for enteral/parenteral nutrition needs and orders nutritional supplements to meet patient's nutritional needs Qualifications Current Registered Dietician license in the state of Maryland Bachelor's degree with a major in Food and Nutrition, Dietetics, or a related fieldAt least 2 years’ experience as a dietitian, preferredKnowledge of the Nutrition Care Process and ability to apply all steps of the processA strong desire to deliver care in an empathetic, compassionate wayPrevious experience working in a skilled nursing facility highly desiredSuperb organizational and time management skillsExcellent interpersonal, written, and verbal communication skillsAbility to counsel and educate othersDecision making, problem solving and collaboration skills a plusStrong leadership skills and ability to work collaboratively with team members
