Clinical Director, Select Health
OverviewWe’re looking for a dynamic and results-driven individual to join our team as a Clinical Director, Select Health (RN or NP). In this position you will develop, plan and direct the implementation of intervention strategies to improve member engagement and clinical outcomes for Select Health members including grant funded efforts directed at achieving HIV viral suppression. If you have a public health background, and interested in healthcare program development in underserved and under resourced communities this may be the role for you. Experience in working with and training community health workers and people with lived experience from impoverished communities preferred. Determines best interventions to serve the HIV population. + Creates assessment and reporting tools to ensure consistent evaluation and tracking of Select Health members. Oversees monthly reporting of progress; analyzes results and makes recommendations for changes, as appropriate. + Trains staff on identifying adherence barriers and intervention opportunities and on engaging Select Health members regarding follow up care. + Monitors and oversees vendors performing outreach and adherence activities and PCP sites serving the HIV population. + Evaluates the quality and appropriateness of services provided by VNS Health Select Health; identifies opportunities for improvement and provides recommendations to leadership. + Organizes and chairs case conferences between the plan and partners. Qualifications Licenses and Certifications: + License and current registration to practice as a Registered Professional Nurse in New York State required + New York State License and current registration to practice as a Nurse Practitioner in New York State preferred Education: + Bachelor's Degree in Nursing or a health care related field required + Master's Degree in business, health care administration, or management preferred Work Experience: + Minimum of five years of progressive health care management experience, including a minimum of three years of experience (working with HIV patients with a focus on public health) required + Strong knowledge of health care delivery systems, managed care, and patient care required + Strong knowledge of Federal and state health care regulations related to Medicare, Medicaid, managed care, and long term care required CAREERS AT VNS Health The future of care begins with you.
