Job Summary Montgomery County, PA Street Medicine program is seeking a Clinical Director based out of Pottstown Hospital. This grant funded position offers an exceptional opportunity to provide impactful health care as the leader of a passionate team of providers, support staff, and community partners. The Clinical Director will be a licensed Nurse Practitioner, responsible for the supervision, management, and delivery of high quality clinical care to the homeless and housing insecure patient population in accordance with State and Federal rules and regulations and the nursing standards of care (assessment of health status, diagnosis, development of plan of care and treatment, implementation of treatment plan, and evaluation of patient status). Develops Street Medicine Consult SOPs. Supervises staff in the provision of direct clinical services. Monitors employee's progress and completes annual performance reviews within hospital designated time frames. Reviews and monitors clinical records to ensure compliance with professional standards. Assists in programmatic administration and development of services, policies and procedures, compliance needs, Continuing Medical Education Committee and others as appropriate. Documents disciplinary actions and provides coaching to staff as appropriate according to hospital guidelines. Completes initial clinical evaluation. Develops treatment plan. Documents in Epic. Completes discharge summary. Orders and interprets diagnostic and therapeutic tests, relative to patient’s age-specific needs; Prescribes appropriate pharmacologic and nonpharmacologic treatment modalities; Monitors the effectiveness of interventions; Collaborates with multidisciplinary team members by makingappropriate referrals.
