*POSITION SUMMARY*
The Clinical Health Coach promotes patient self-management by encouraging direct communication with primary care providers and empowering active participation during care transitions. This role involves guiding patients in lifestyle modifications, setting health goals, and providing follow-up for sustainable behavior change. The Clinical Health Coach offers wellness coaching to reduce high-risk behaviors, enhance chronic condition management, prevent hospital admissions/readmissions, and improve overall patient well-being. The Clinical Health Coach collaborates with hospital leadership, multidisciplinary teams, and quality management to identify process improvements and optimize care transitions across the continuum.
*PRIMARY ACCOUNTABILITES *
1. Works with care teams and technology to identify high-risk, high-need patients by helping to implement best practice processes for chronic care and disease management (CHF, AMI, COPD, PNA, CABG and TKR/THR), while providing patient education and referring patients to available health resources when appropriate.
2. Acts as the liaison between the patient, the patient support network, treating physician(s), and ancillary providers utilizing a multi-disciplinary approach to assess, identify, plan, health goal setting and measure optimal clinical outcomes.
3. Utilizes Motivational Interviewing to assess readiness, health goal setting short and short and long-term needs; Utilizes strategies to engage patient's plans for change that follow standard policy and procedures, clinical guidelines and national best practices.
4. Demonstrates effective communication with other members of the healthcare team and promotes effective team functioning.
5. Provides service to patients and families with sensitivity and respect for their needs, expectations, age, cultural, and individual differences.
6. Works with the physician advisor, physicians, nursing, ambulatory programs, outpatient programs, interdisciplinary team and health plan for defined patient populations to develop clinically appropriate transitional pathways, continuum care management for patient care and patient satisfaction.
7. Undertakes additional responsibilities as assigned to support departmental operations and organizational objectives.
*MINIMUM QUALIFICATIONS*
* Education: Bachelor’s degree in health-related field.
* Work Experience: Five (5) years’ experience in Motivational Interview and Clinical Knowledge.
* Licensure: None
* Certification: None
* Work Experience In Lieu of Education: Licensed Practical Nurse or Medical Assistant Licensure and Three (3) additional years’ experience.
* Skills/Knowledge/Abilities:
o Proficiency in Microsoft Office – Outlook, Word, Excel, PowerPoint, etc.
o Skilled in managing multiple projects and priorities, adapting approaches to achieve goals in changing situations, and handling service issues professionally.
o High emotional intelligence and self-awareness, ensuring confidentiality and sensitivity in patient and organizational matters.
o Expert knowledge in disease and lifestyle topics, including weight management, tobacco cessation, and chronic conditions (CHF, COPD).
o Proficient in healthcare practices, office workflows, and medical terminology, with experience in community education and public speaking.
o Skilled in motivational interviewing, with a strong understanding of behavior change theories and their practical application.
o Strong verbal and written communication skills, effectively using motivational interviewing techniques and conducting educational/support groups.
o Capable of working independently, prioritizing a heavy workload with minimal supervision, and communicating timely with all stakeholders.
o Ability to occasionally work weekends and holidays as needed.
*PREFERRED QUALIFICATIONS *
* Certification: Health coaching certification by a NCCA accredited organization, or Wellcoaches coaching certification.
*PHYSICAL REQUIREMENTS*
* Majority of time involves sitting or standing; occasional walking, bending, and stooping.
Long periods of computer time or at workstation.
Light work that may include lifting or moving objects up to 20 pounds with or without assistance.
May be exposed to inside environments with varied temperatures, air quality, lighting and/or low to moderate noise.
Communicating with others to exchange information.
Visual acuity and hand-eye coordination to perform tasks.
Workspace may vary from open to confined, onsite, or remote.
May require travel to various facilities within and beyond county perimeter; may require use of personal vehicle.
