The Health Coach responsibilities include:

Using a personalized approach to healthcare that combines principles from both traditional medicine and coaching to help individuals achieve their health and wellness goals. Utilizes a collaborative process to support clients in making sustainable lifestyle changes, managing chronic conditions, and improving overall well-being. Works with individuals and groups to facilitate and empower them to develop and achieve self-determined goals related to health and wellness. Supports individuals in mobilizing internal strengths and external resources and in developing self-management strategies for making sustainable, healthy lifestyle, behavior changes based on their own goals and consistent with treatment plans as prescribed by individual’s professional health care provider as it relates to disease management and prevention.Develops, coordinates, and participates in other activities and responsibilities as required by the Community Health dept.

Required License/Certification includes BLS, AED, and one of the following Nationally Recognized Credentials within 3 months of hire; Clinical Health Coach, ACE, or WellCoaches certification.

This position requires AHCA Level 2 background clearance and requires travel within home base county but also to our sister counties with your own reliable transportation.
