Description
Reporting to the Clinical Intake Manager, the Clinical Intake Coordinator is responsible for timely and accurate data entry of all hospice referrals into the electronic software system; tracking all pending referrals until admission or non-admission; obtaining medical records; sending electronic consents and instructing POA in completion of consents. The Intake Clinical Coordinator will play a vital role in reviewing clinical records to ensure the RN scheduled to assess the patient's eligibility is set up for success with proper documentation and clinical records to support the admitting hospice diagnosis. The shift is Monday-Friday 10:30AM - 7 PM.
Essential functions of this role include:
+ Receives patient referrals via fax, phone and email and enters into HCHB in a timely manner.
+ Establishes and maintains a positive working relationship with current and potential referral sources.
+ Facilitates and schedules the nurse evaluation by coordinating with patients, families, facilities, and branch staff.
+ Obtains medical records via fax requests and / or via other health organizations EMR system.
+ Reviews referrals for clinical needs to assist and prepare the RN for the evaluation.
+ Coordinates hospital discharge ensuring a smooth transition to patient's home .
+ Orders DME and other supplies when appropriate.
+ Assists with referral management including scheduling evaluations to ensure timeliness to the patient's bedside.
+ Addresses and resolves any barriers to admission.
+ Maintains all updates for pending referrals.
+ Reviews DocuSign paperwork with patient's POA if they are unable to be at the evaluation.
+ Accountable for compliance with all HIPAA, CMS, CHAP and CARPS regulations and standards.
+ Performs other related duties as assigned.
+ Responsible for maintaining compliance with all HIPAA regulations and requirements. Treats all patient information as confidential and utilizes protected health information on a minimum necessary basis.
Qualifications
Knowledge and Skills required:
+ Graduate of an accredited school of nursing. At least three (3) years of hospice experience. LPN or RN licensure required.
+ Three (3) + years experience in healthcare setting and understanding of medical terminology.
+ Strong computer skills including advanced knowledge of Microsoft Word, Excel and PowerPoint.
+ Strong organizational skills and an ability to prioritize and complete simultaneous projects with minimal supervision.
+ EMR and HomeCare HomeBase experience preferred
+ Excellent written and oral communication skills.
+ Ability to pass DHS background study.
+ Proficient in the English language. Able to read, speak, and write English in a clear and concise manner required.
The hourly base range for this role is$28.00-$31.00 annual base salary. An employee’s actual annual salary will be based on but not limited to: geographic location, relevant education, applicable certifications, depth of experience, and special skillsets.
