Clinical Liaison, LVN

The Clinical Liaison position is responsible for creating and maintaining positive customer relationships in order to increase referrals for Home Health, Hospice, Home Infusion, PACE, Transitional Care Unit and other services within assigned accounts.  This role includes working with discharge planners, facility staff, physicians, and individuals to assess appropriateness for hospice and home health services, and facilitate the coordination of services and to ensure there is appropriate documentation for compliance with agency and statutory guidelines to manage the transition of the patient from an acute care facility to the patient's home or next location of residence.

Required Qualifications:

Education sufficient for clinical licensure for LPN/LVN discipline  Current state LPN/LVN license 3 years experience as LPN/LVN

Preferred Qualifications:

Liaison, marketing or sales experience. Documented prior successful sales account medical management experience. Epic EMR experience.
