Join our team as a Clinical Liaison where you’ll play a key role in connecting with referral partners and prospective patients. You’ll help ensure a steady flow of appropriate referrals , support census goals, and guide patients through the admission process alongside hospital and Center staff. Experience + Proven background in Skilled Nursing, specifically in screening patients for potential admissions. + Familiarity with a variety of Hospital EMR systems, including Allscripts, 4Next, NaviHealth , and Epic. + Responsible for overseeing the full scope of the referral and admission process from start to finish. + Skilled in receiving and evaluating referrals to determine clinical appropriateness for hospital admission. + Collaborate closely with hospital teams to ensure all components of the admission process are completed thoroughly and accurately.
