CHASI serves the most vulnerable individuals, families, and communities with critical services and programs. CHASI provides outreach, education, prevention, and direct support services for populations most affected by health disparities – the poor and working poor, low income people with chronic illnesses, persons with criminal justice involvement, substance users, domestic violence survivors, people of color, and the LGBTQ community. POSITION SUMMARY: CHASI’s Prevention & Intervention Department focuses on seeking out and coordinating education and referrals for persons within priority populations to decrease acquisition of HIV and STIs through comprehensive effective strategies. The Playsure initiative is designed to universalize and normalize HIV prevention conversations to eliminate stigma and ensure client choice. As a program that strives to address the social and structural determinants of health of our clients, the Clinical Manager will play a key role in the delivery of mental health, substance use, and other supportive services. They will also supervise the program’s Navigators. DUTIES & RESPONSIBILITIES: + Provide regular supervision to and evaluation of the program’s Navigators, including their use of the funder-required assessment tools (example: PHQ-9 and CAGE); manage personnel needs, including collaborating with Human Resources and department leadership on recruitment, orientation, evaluation, and corrective action, as needed + Conduct comprehensive assessments and determine appropriate level of care + Maintain a case load and provide short term mental health and SUD counseling for enrolled Playsure clients from priority populations + Provide crisis intervention, de-escalation and support to clients as needed + Support identification of and referral to other support services during individual client sessions + Increase availability of, and improve access to, cultural and linguistically affirming mental health care + Be familiar with internal and external services in order to provide appropriate referrals to clients; build relationships and work collaboratively with partner providers + Work with clients to ensure connection and compliance with appointments and their agreed upon action plan and address barriers. + Collect and submit data to CHASI’s Data Unit accurate data on persons contacted and engaged in services. Maintain accurate records on the services and activities provided. Participate in documentation quality assurance and reconciliation tasks + Keep up to date on a weekly basis with the program client list and deliverable tracker + Monitor client progress and maintain detailed, accurate and complete documentation in a timely manner + Work closely with the Director, HIV Prevention on reviewing charts for compliance, accuracy, and quality + Participate in training, workshops, professional development activities, and meetings, as assigned including CHASI management team + Participate in training, professional development activities, and meetings, as assigned + Perform all functions in alignment with CHASI’s Mission, Vision, and Core Values + Maintain appropriate positive, professional relationships with staff, peers, and clients + Perform other duties as assigned and participate in special projects, as required. Requirements QUALIFICATIONS: + Master’s degree in Social Work or Mental Health Counseling focused on Individual and Family practice, with eligibility for licensurerequired. + At least 3 years of experience providing counseling and/or case management with individuals at high risk of contracting HIV, or related work experience. + At least 1 year of experience supervising staff + Bilingual (English/Spanish)required. + Must have basic knowledge regarding HIV, HCV, STI, PEP/PrEP and Harm Reduction methods. + Excellent MS Office skills (Outlook, Word, Excel, PowerPoint). + Ability to work independently and also be team player with an ability to work with diverse teams with shared responsibility and accountability; ability to contribute to a positive, productive, open, and supportive environment that motivates others to perform at their best. + Must possess ability to interact with all levels of external and internal partners with a high level of professionalism and tact in representing CHASI. + Strong verbal and written communication skills, including capability to listen and follow instructions; able to give, provide direction and follow up on tasks; high attention to detail and accuracy.
