The pay for this position ranges from $39.25-61.36.
Position Summary:
The Clinical Manager is responsible for managing the nursing clinical activities of the agency; planning clinical services to patients and conferring with caregiver to assure services are provided at quality levels consistent with professional standards of practice. The position reports to the Director of Patient Care Services.
Qualifications:
•Currently licensed as a R.N. in the state of New York. The license must not have been revoked, suspended, and without limitations or restrictions.
•Have at least two (2) years experience in the home care field.
•Demonstrate ability to make appropriate nursing judgements.
•Strong interpersonal communication and teaching skills.
•Strong organizational skills and the ability to work independently.
•Strong written and oral communication skills.
•Access to a dependable vehicle in order to travel to multiple organization business stops a day in all types of weather.
•A valid driver's license and proof of car insurance when using a personal vehicle for organization business.
•Current BLS.
•Preferred: OASIS certification
Essential Functions:
Patient Care:
Assists in ensuring patient care is delivered according to established care guidelines, Plan of Care, physician orders, agency policies and procedures and meets insurance, state, federal and payer requirements.
Establishes a positive working relationship with the interdisciplinary leadership team and direct patient care teams.
Plans, directs and participates in the delivery of patient care services, including case assignment and visit scheduling activities.
Monitor nurse scheduling; assure caregiver qualifications are matched to patient needs.
Establishes and implements priorities for service, when necessary.
Recommends, implements and interprets policies and procedures relevant to the provision of patient care services.
Serves as a clinical resource to caregivers.
Participates in the intake and referral process to receive complete clinical information and assure patients are accepted for care according to organization admission criteria.
Reviews/audits clinical record documentation to assure compliance with state and federal regulations and organization policies and procedures.
Performs in person and virtual supervisory visits.
Participates in the Home Health Quality Program
Communicates effectively with Case Managers and caregivers to monitor the changing needs of the patient and the family.
Leads nursing meetings and case conferences.
Participates in the review of OASIS assessments and Plan of Care Reviews ensuring that care needs align with the Plan of Care established.
Participates in the support of the rehab medication reconciliation practice
Participates in the after-hours on-call process to ensure patient care policies and procedures are followed and staffing issues resolved.
Supports nursing census and patient care needs by performing SOC assessments or other type of nursing visits of on average of at least weekly, excluding supervisory visits.

Community Relations:
Acts as a liaison with other community organizations, to include community education.
Serves as a liaison between the organization and the patient and family.
Promotes a positive image of the organization services in the community.
Develops and maintains a good working relationship with referral sources including but not limited to physicians, hospitals, nursing homes, and other community resources.

Personnel Management:
Assists with the recruitment and hiring of qualified caregivers to meet patient Nurse Clinical Manager care needs.
Assists with the orientation of new caregivers and Home Health Competency Evaluation according to established guidelines.
Maintains and enhances professional and managerial expertise through appropriate education, in-service and training activities; identifies caregiver learning needs.
Assists with the performance review process of all organizational caregivers.
Establishes an environment that fosters the development of professional and paraprofessional caregivers.
Monitors the skills and services of caregivers.
Maintains on-going communication with caregivers.
Review staff timecards for accuracy
Approval of PTO requests to ensure staffing needs will be met
Coordination with scheduler to navigate difficult staffing situations, such as call off or unexpected increased patient care needs.
Other duties as assigned
Communicates with the schedulers with regard to the scheduling needs and concerns of patients and families.
Portrays role modeling, teamwork, accountability and a positive attitude.
Keeps Director Manager informed of all critical patient care information.
Coordinates Nursing Schedules for daily staff coverage, weekend coverage, monitoring PTO approvals to ensure staffing is adequate for the delivery of care.

Assists the Director in controlling costs to achieve organizational financial objectives.
Assists the Director in monitoring nursing field caregivers productivity.
Works closely with the Director to ensure gross profit objectives for each patient are met, ultimately meeting the organizational financial objectives.
