Job Summary: The Clinical Nurse C-PD Dialysis, is an experienced registered nurse and functions primarily in the outpatient clinic environment serving patients of all ages but also sees admitted patients as needed. Exercises discretion and independent judgment in providing care for peritoneal dialysis patients of all ages. Practices in a patient-centered care model of practice and performs the nursing process independently. Ensures patient information is available for review with physicians and the multidisciplinary healthcare team at time of patient's clinic visit. Educates and empowers patients and caregivers to administer dialysis in their own home. Makes home evaluation visits prior to the start of peritoneal dialysis and at designated intervals thereafter to evaluate the home environment for safety, space and treatment set-up to prevent complications related to the home environment. Reports findings to the physician and multidisciplinary healthcare team. Complies with all federal and state End-Stage Renal Disease (ESRD) regulations. Serves as a liaison and consultant to inpatient peritoneal dialysis services for Loma Linda University Health (LLUH) multi-hospital system and academic medical center. Serves as a role model in patient care delivery methods and consistently demonstrates an advanced level of clinical practice, communication and documentation. Applies theoretical concepts to clinical practice and continually expands knowledge base and clinical skills with a focus in end-stage renal disease. Promotes the empowerment of the nursing profession and excels in implementing new knowledge, innovations and improvements in practice to enhance the patient experience and clinical outcomes. Participates in on-call schedule. Performs other duties as needed.
Education and Experience: Bachelor of Science in Nursing (BSN) required (degree may be in progress with expected completion within 24 months of hire). Minimum two years clinical nursing experience in dialysis required. Must meet standards outlined in the Clinical Nurse Ladder handbook. New hires are granted one year to meet CNC standards outlined in the Clinical Nurse Ladder handbook.
Knowledge and Skills: In-depth knowledge of end-stage renal disease treatment modalities. Demonstrates advanced competencies in dialysis clinical nursing. Ability to apply theoretical concepts to clinical practice and continually expands knowledge base and clinical skills with a focus in dialysis. Basic knowledge of healthcare regulations and accreditation required. Exhibit professional behavior at all times. Licensures and Certifications: Active California Registered Nurse (RN) license required. American Heart Association Basic Life Support (BLS) certification required. Other certifications as outlined in the department Plan for Providing Care.
