Clinical Nurse Coordinator-Department of Orthopedics

Position Highlights:

Position: Clinical Nurse CoordinatorHourly Pay Range: $39.78 - $67.72. The hourly pay rate offered is determined by a candidate’s expertise and years of experience, among other factors.Location: Skokie, ILFull Time/Part Time: Full Time, 40 hours per week, day shiftHours: Monday-Friday, 8:00a, start time may occasionally start earlierRequired Travel: as needed to Glenbrook, Highland Park, Evanston and Swedish

What you will need:

Bachelor’s Degree or equivalent in Nursing or Social Work; Master’s degree in a professional discipline, business, or other health field preferred3 years’ experience in a clinical setting providing care management, discharge planning services to patients (either acute or post-acute)Strong interpersonal and effective written and oral communication skillsProven leadership skillsAbility to work independently, problem solving and setting priorities to achieve results

What you will do:

A. Assures Professional Continuum Care Manager and Social Work practice is delivered to provide highest level of care to patients

Accountable for the delivery of high quality patient care within CCC departmentAssures the implementation of clinical and CCC policies and proceduresAssures the establishment of standards of care that are consistent with those of the community and the profession.Assists with staff development to promote professional growth and education for the staff and the CCC Department

B. Develops staff for clinical competence

Acts as an on-site resource to clinical team members to respond to clinical issues and resolve clinical problems.Implements change impacting personnel, processes and procedures to meet corporate and departmental goals.Provides input to the Value Based Care Manager on interim and annual performance evaluations regarding individual clinical competence and observed performanceProvides support to staff completing orientation in collaboration with Manager of Value Based care and assigned staff preceptor.Continuously evaluates staff performance and provides constructive feedback to staff in order to promote their ability to successfully achieve planned clinical outcomes.Holds staff accountable to adhere to national care management standards of practice.

C. Provides staff leadership to support CCC staff and department operational effectiveness

Meets regularly with Value Based Care Manager to discuss issues / concerns and plan for follow up resolutionMakes recommendations to improve workflow issues and clinical performanceCooperates with manager to implement staffing schedules and daily assignments to meet patient care demand and budget.Assumes delegated responsibility to monitor staff attendance and manage PTO to adhere to organizational policies.Assigns patients to social work and nursing case managers in order to provide consistency and continuity for patient care within the format of integrated model (dyad) care management service
