Position Summary: Clinical Pharmacist, Ambulatory Care (East Beltline & Browning Claytor Offices)
Responsible for providing clinical pharmacy services in an outpatient and/or inpatient practice area, by providing drug therapy management and assessment for patients in the practice area. Provides support for established programs including, but not limited to, drug formulary, pharmacy education programs, and quality improvement and research relating to drug therapy.
What you will need:
Education/Training: Doctor of Pharmacy degree (PharmD) from an ACPE-accredited school/college of Pharmacy (required), PGY-1 Residency (required), PGY-2 Ambulatory Care Residency (strongly preferred) or commensurate practice experience in ambulatory care with direct pharmacotherapy management responsibilities; BPS Specialty Certification (Pharmacotherapy or Ambulatory Care, preferred; or eligible with certification within 1 year of hire)
Licensure: Licensed (pharmacist and controlled substance) in the State of Michigan, or eligible
Description: This is a full-time (80 hours per pay-period), salaried, Monday thru Friday position for a Clinical Pharmacist, Ambulatory Care within the Trinity Health Medical Group primary care, PCMH offices (part of the Trinity Health Grand Rapids health-system in Grand Rapids, Michigan). Responsibilities include disease management within institution approved collaborative practice agreements (diabetes, hypertension, hyperlipidemia, asthma, COPD, anticoagulation, others), referral-based Comprehensive Medication Review with recommendations to physicians, Transition of Care visits, daily ad hoc informal consultations to improve pharmacotherapy effectiveness, safety, and cost as well as participating in team-based care within a multidisciplinary model. Additional activities include time in our Anticoagulation Management Service (cross-trained to allow for coverage on an as needed basis), antimicrobial stewardship partnership with our ID team, & pharmacy unit inspections. Participation in the training of Trinity Health Grand Rapids PGY-1 & PGY-2 Ambulatory Care Pharmacy residents and PharmD students from the College of Pharmacy at Ferris State University is also expected as is participation in both medical group and/or health system committees.
