**Responsibilities** The **Clinical Pharmacist** provides comprehensive pharmaceutical care services to patients, working collaboratively with physicians, nurses, and other healthcare professionals to optimize medication therapy and improve patient outcomes. The **Clinical Pharmacist** is responsible for medication order review, pharmacokinetic monitoring, drug information provision, patient education, and participation in clinical initiatives. + Review and verify medication orders for appropriateness, safety, and efficacy, considering patient-specific factors such as age, weight, renal function, and allergies. + Monitor serum drug concentrations and adjust dosages as needed to achieve therapeutic goals and minimize toxicity. + Provide accurate and timely drug information to physicians, nurses, and other healthcare professionals. + Educate patients on their medications, including proper administration, potential side effects, and drug interactions. + Conduct comprehensive medication reviews and provide MTM services to patients with chronic diseases. + Participate in antimicrobial stewardship programs to promote the appropriate use of antibiotics and prevent antimicrobial resistance. + Monitor for and report adverse drug reactions. **Qualifications** + Completion of a 4-year course at an accredited School of Pharmacy with a BS or Pharm.
