We are looking for a dedicated Clinical Pharmacist like you to be a part of our team. **Job Summary and Qualifications** + Leads committees responsible for establishing and implementing medication-related policies and procedures (e.g. Pharmacy & Therapeutics, Medication Safety, Antimicrobial Management Program) + Is primarily responsible for the direction of one or more service lines with respect to pharmacy service lines (e.g. cardiovascular, pediatrics, critical care, etc.) + Demonstrates leadership within the pharmacy department through a documented record of improvements in and contributions to pharmacy practice + Maintains ongoing commitment to advancing the profession in the area of pharmacy practice in which they serve + Provides regular, formal education to pharmacy and interdisciplinary teams in their field of expertise + Models, mentors, and encourages pharmacy students, residents, and colleagues in a manner that fosters highly professional pharmaceutical care and high personal standards of integrity + Prepares and dispenses medications, including sterile, chemotherapy, and parenteral nutrition preparations safely and accurately using appropriate techniques, USP 797 & 800 guidelines + Evaluates Total Parenteral Nutrition (TPN) for appropriateness based on facility policies and guidelines or ASPEN guidelines or recommendations as appropriate + Issues controlled substances to patient care areas and maintains records as required by law and follows corporate policies and procedures related to the dispensing and monitoring of controlled substances **What qualifications you will need:** + Pharmacist + Bachelors Degree, or Doctorate Degree, or Pharmacy Practice Residency - PGY1 + 5 years of hospital experience preferred The advanced clinical pharmacist practices the profession of pharmacy with the utmost code of ethics demonstrating the organization’s mission and values. The clinical pharmacist is responsible for the safe, effective, evidence-based, and cost-effective selection and utilization of medications for the patients in which they serve. The clinical pharmacist uses advanced knowledge, skills, and professional judgment in all aspects of his or her work. The clinical pharmacist serves as a patient advocate when planning and implementing pharmaceutical care within a multidisciplinary rounding team, collaborating with other healthcare professionals in a knowledgeable and professional manner.
