Position Summary: Compounds and dispenses prescribed medications, drugs, and other pharmaceuticals for patient care by performing the following duties personally. Responsible for ensuring that all critical tasks are fulfilled on a timely basis.
Critical Tasks:
Maintains exclusive focus upon patients, the quality of their care, and their individual needs without significant distraction by non-patient contact activities and related position duties.Reviews prescriptions issued by physicians or other authorized prescribers to assure accuracy and determine formulas and ingredients needed. Provides pharmaceutical services focused on therapeutic drug monitoring.Compounds medications, using standard formulas and processes such as weighing, measuring, and mixing ingredients.Directs pharmacy workers engaged in mixing, packaging, and labeling pharmaceuticals.Answers questions and provides information to pharmacy customers on drug interactions, side effects, dosage and storage of pharmaceuticals.Maintains established procedures concerning quality assurance, security of controlled substances, and disposal of hazardous waste drugs.Enters data such as patient name, prescribed medication and cost, to maintain pharmacy files, charge system, and inventory.Assays medications to determine identity, purity, and strength.Instructs interns and other medical personnel on matters pertaining to pharmacy subjects.Prepares intravenous (IV) packs according to prescriptions and instructions.Prepares sterile products while performing all such tasks in the Clean Room using proper aseptic technique and while complying with Sterile Room policies.Prepares and packages prescription medicines and other medical products with 100% accuracy for delivery to patients. Accurately retrieves, counts, pours, and affixes appropriate labels.Provides accurate, up-to-date and timely drug information to the professional staff as well as to patients and their caregivers. Serves as a therapeutic consultant to the medical staff and provides education and in-service training to medical staff, pharmacy, and nursing staff as requested.Confirms and reconfirms all patient information, prescription data, medication dosage and frequency, all other drugs used by patients, all possible adverse reactions, and all recent drug utilization histories of patients to ensure patient safety and proper/intended medicinal effects of drugs prescribed. Actively participates in continuous quality assurance and performance improvement activitiesTakes all steps necessary to fully ensure patient/resident and medical data confidentiality and security by all staff.Complies fully with all patient/resident records and information privacy regulations, state and federal.Supports the formulary development process by consulting with medical staff and promoting rational drug therapy selection, and reviews insurer information to confirm reimbursement ability.· Performs disease state management services while recommending addition or deletion of drugs based on disease state. Reviews and interprets pertinent lab data required for medication monitoring. Recommends ordering of laboratory tests if necessary.Recommends first line formulary drugs for initial therapy based on safety, efficacy, cost and side effect profiles.Counsels patients, families, and caregivers on the use, storage, cautions and side effects of all medications dispensed as well as cognitive services provided. Analyzes medication regimens to ensure the most effective, least toxic, and most economical treatment plan is implemented successfully.Answers telephones and responds to questions and requests. Complies to a degree of 100% with all pharmacy laws and regulations particularly as they relate to DEA controlled substances.Counts stock of controlled substances and enters data into computer to maintain fully accurate inventory and dispensing records.Demonstrates ability to regularly change priorities to accomplish all tasks despite frequent interruptions.Demonstrates ability to clearly communicate, both orally and in writing, while performing all essential functions.Performs other special projects and duties as assigned.Required Qualifications:
Bachelor’s Degree in Pharmacy from an accredited university, with Doctorate preferred.One year of pharmacy residency completedArizona Pharmacy License.Continuing Education credits received as per Arizona State Board of Pharmacy requirements.Two to ten years of Licensed Pharmacist experience within a hospital pharmacy, with at least two recent years performed in Arizona.Demonstrated ability to relate to diverse cultures and specifically the Gila River Community and/or other Native American cultures.
Prior medical environment work experience is preferred.
Interest in medical subjects, patient services/care, and customer service.
