Clinical Pharmacist II
As our Clinical Pharmacist in you will be responsible for providing clinical pharmacy services to meet the needs of patients and support the mission of the Department of Pharmacy, Hospital, and Health System. Provides comprehensive pharmacy care through protocol/guideline management as well as individualizes drug therapies to promote safe, efficacious and cost‐effective patient care.

You'll build trust in these interactions by reviewing every prescription in your queue. We never rely entirely on the computer to check for safety; we always use an expert for the final check. That’s you. You'll review prescriptions for potential drug interactions, correct dosing, patient allergies or anything that looks unusual in your opinion.

Responsibilities
Develops and Provides educational programs, learning modules, and knowledge assessment tools.Reviews, interprets, and evaluates all medication ordersConsults with and advises medical, nursing, and other healthcare providers regarding the prescribing, dispensing, or administration of medicationsRoutinely monitors the patient’s total drug therapy for effectiveness, side effects, adverse reactions, toxicities, drug interactions, and incompatibilities
Qualifications
Required Education and Experience
Registered Pharmacist with the Texas State Board of Pharmacy in good standing‐(RPH) Aseptic Technique – Complete an ACPE approved 20 hour sterile products course within 90 days of hire Post Graduate Year 1 (PGY1) Residency or 3 years of experience
