Clinical Pharmacist

Position Performance Area/Expectations: 

Quality

Reviews and assesses adverse drug reactions for opportunities to improve prescribing patterns or processes.  Completes Adverse Drug Reaction Form. Reviews medication variances for opportunities to improve the medication use process.  Completes Medication Variance Report Form. Designs and conducts concurrent and prospective evaluation of drug therapy through efficient and effective drug utilization evaluations. Ensures that all drug storage areas are inspected according to schedule so that no outdated, recalled or used items are present and that all stock items are properly labeled and stored (i.e. crash carts, floor stock, Pyxis). Ensures that inventory in the pharmacy and all patient care areas that are supported by pharmacy services meets defined levels. Demonstrates proficiency in the safe preparation of all IV admixtures. Adheres to safety policies and procedures. Keeps abreast of regulatory standards and ensures compliance through assisting in the establishment and implementation of policies and procedures. Participates in annual self-assessment of skills.  Meets goals for areas previously defined as needing reinforcement.

 

Clinical

Clarifies any drug therapy questions with physician and nursing staff immediately. Maintains a primary focus on sound pharmacoeconomic principles by assisting the physician in his/her therapeutic decisions, making appropriate formulary interchanges, performing age / weight / renal function / comorbid condition appropriate dosing adjustments, which optimize drug therapy. Documents therapeutic interventions. Maintains and participates in the ongoing review of the drug formulary through Pharmacy and Therapeutics Subcommittee assignment. Maintains current BLS Certification and responds to CPR emergencies within unit-based assignment.

 Dispensing

Provides feedback regarding technicians’ performance and suggests opportunities for improvement. Ensures that all orders are dispensed according to turn around time standards. Ensures all medications are dispensed and documented accurately, as well as checked for compatibility, and labeled appropriately. Verifies all medication records, including controlled substances records, are accurate, complete and maintained according to policy.  Reconciles any discrepancies immediately. Has authority to transport medication(s) throughout organization including off campus locations

Education

Ensures patients/family are educated regarding their drug therapy and education is documented in the patient’s chart.

Patient Population this Position Services:  Neonates, infants, children, young adults, adults and geriatric patients.

Position Requirements:

Current Illinois license for registered Pharmacist. PharmD, or Bachelor of Science degree in Pharmacy with acute care experience as a pharmacist in an inpatient hospital pharmacy setting with clinical exposure. Two - three years advance clinical experience preferred. Board Certified Pharmacotherapeutic Specialist preferred. CPR certification within 2 months of hire Good communication skills. Good organizational skills.
