Clinical Pharmacist
**Job Description:** The Clinical Pharmacist demonstrates consistent application of knowledge and skills to deliver appropriate, safe, efficient, effective, and cost effective pharmaceutical care in the scope of their particular assignment. This position is responsible for the care of patients as well as the development of effective, professional relationships within the multidisciplinary team. 1.Coordinates and oversees clinical pharmacy services related to neurology at the assigned clinic. Serves as a pharmacy team leader and expert clinician within defined area of practice. Assists other staff with the development of care plans for patients. Provides direct clinical pharmacy services to selected patients. Ensures safe, appropriate, cost effective medication therapies for patients. Uses specialty pharmacy software, pharmacy dispensing software, and the EMR to document all care provided to patients. 2. Coordinates and develops the Specialty Pharmacy program at the assigned neurology clinic in conjunction with the Specialty Pharmacy Manager. Supports the care site neurology service in conjunction with the site Pharmacy Director. Confers with the Specialty Pharmacy Manager in planning both short and long-term annual budgets for operations and capital for services. 3. Works as a cohesive team with the clinic specialty pharmacy technician, the Franklin Pharmacy specialty pharmacists and specialty pharmacy technicians, and the SCL Health Pharmacy Call Center to provide a seamless pharmacy experience for patients of SCL Health. 4. Works collaboratively other staff to create clear, efficient, and well documented workflows. Develops effective and collaborative relationships with Physicians and other independent Licensed Providers to support pharmacy's continued participation in the Neurology Service. Consults with members of the medical staff on orders for non-formulary drugs to offer formulary alternatives when possible. 5. Enhances quality of delivery of care. Participates in the development and implementation of performance/quality improvement and patient safety activities for pharmacy hospital and clinics. Identifies potential technical or patient care issues and accepts accountability for follow-up and resolution of issues. Serves as a liaison between patient care areas and pharmacy staff/ managers. 6. Assists in the development and implementation of staff competencies both within service area and for general pharmacy services. Participates collegially in the development of new programs, services and practices, and educational activities of the pharmacy department and with the multidisciplinary team within defined area of practice. Provides in- services/lectures for Pharmacy Department and for health care professionals in training (medical, nursing, pharmacy). Serves on committees and work groups. 7. Participates in Resident Faculty meetings, mortality and morbidity rounds, grand rounds, and resident lecture series within defined area of practice. Establishes meetings with the Medical Director and Nurse Manager of defined service line. Participates in multidisciplinary and departmental research initiatives and stays informed on all investigational drug studies being performed within their defined area of practice. 8. Participates in Performance Improvement activities and initiatives. Devises methods to track impact of new approaches. Identifies high- cost and high-use agents within defined area of practice and participates in financial management initiatives. Uses appropriate resources throughout the organization to meet customer needs. Responsible and efficient utilization of human and material resource. 9. (All SCL jobs) Performs other duties as assigned. **Minimum Qualifications:** + Graduate of an accredited College of Pharmacy with a Pharm D Degree or a five year Bachelor of Science Degree in Pharmacy + Licensure in the state of practice (as appropriate) to practice pharmacy Qualified to practice Drug Therapy Management under the Rules + Regulations of the practice state **Preferred Qualifications:** + PGY1 pharmacy residency or fellowship + Board Certified Pharmacotherapy Specialist (BCPS) or Board Certified Ambulatory Care Pharmacist (BCACP) + Five (5) years of experience in neurology practice in a hospital or ambulatory setting To perform this job successfully, an individual must be able to perform each essential duty satisfactorily.
