Clinical Pharmacist Ambulatory Care
Job Summary: Provides ambulatory based medication management services in collaboration with physicians and providers for patients at high risk of medication-related problems to reduce adverse drug events, improve patient safety, and optimize medication use and health outcomes. Engages with Service Lines, Providers, Care Navigation leadership and staff, and other Health Care Providers to identify and resolve medication related problems and other duties as assigned. Provides patient-centered approach to care. Provides pharmacists’ services/support to managed care contracts and government programs to promote high value care. Advises and provides resources for medication access and other medication-related services as appropriate. Must be knowledgeable and able to utilize efficiently Microsoft Programs, Electronic Health Record (EHR), Disease Registries, etc. necessary to perform the job role. Willingness to travel to offices as needed to meet provider / department / patient needs. Qualifications: Bachelor of Science degree (BSPharm) in pharmacy or Doctor of Pharmacy Degree (PharmD) required. NC pharmacist license required. SC pharmacist license preferred. Completion of accredited PGY1 residency program or minimum 3 years previous experience as a Clinical Pharmacist in a hospital setting or community-based pharmacy required. PGY2 Ambulatory Care preferred. Board certification required within 12 months of meeting eligibility criteria. Clinical Pharmacist Practitioner (CPP) required within 12 months of meeting eligibility criteria with successful credentialing through the medical staff / NCBOP. Thorough knowledge of pharmacy terminology, procedures, and policies; Effective communication expected with patients, families, and other members of the Health Care Team with excellent organizational skills, problem solving/critical thinking, interpersonal, customer service, teaching, coaching, and proficiency in Microsoft Programs. Multi-tasking and time management is required. Ability to build cohesiveness and successfully execute planned goals and objectives to support pharmacy quality measures and pharmacy team responsibilities. Superb provider relation skills and proven ability to engage providers by providing valuable insight and logical information in a clear and concise manner. Analytical skills necessary to interpret quantitative and qualitative data relevant to the market, organization, and quality contracts. Ability to lead and work effectively within groups, the organization, and to support CaroMont mission, vision, and values. BLS and ACLS certification is required within 120 days of start, or first available course offered by CaroMont, whichever occurs later. EOE AA M/F/Vet/Disability
