Clinical Pharmacist - Emergency Medicine

Job Responsibilities 

The Emergency Medicine Pharmacist will provide support to the Emergency Department (ED) and offer patient-centered services in a ED setting. The Emergency Medicine Pharmacist will participate in multidisciplinary committees and serve as a preceptor and educator for other healthcare and non-healthcare individuals including the PGY-2 Emergency Medicine residency.

Additional responsibilities include:

Provide comprehensive pharmaceutical care to our patients including: clinical intervention; teaching and pharmacy representation on various committees; participation in daily patient care; medication order clarifications with the prescriber, document any changes in patient and pharmacy records and inform others of medication order changes; facilitate transitions of care for patients from inpatient to outpatient settings; monitor drug therapy to evaluate appropriateness of use, dose, dosage form, regimen, route, therapeutic duplication, and drug interactions, provide pharmacokinetic consultations as required; prevent, detect, monitor, document, and report adverse drug reactions and medications errors; participate in the drug therapy management of medical emergencies; promote the use of the formulary by converting non-formulary orders to formulary when possible. Obtain patient medication histories; conduct patient education and counseling on medications as well as other agents; facilitate the obtaining of outpatient maintenance medications as needed. Augment the education of pharmacists, prescribers and nurses on procedures and treatments related to emergency medicine patients; serve as a preceptor and mentor for pharmacists, pharmacy residents and students Participate in multi-disciplinary committees including Pharmacy & Therapeutics, etc. to support the appropriate use of medication therapy; conduct medication use evaluations and research projects in collaboration with other healthcare professionals to identify areas of medication use optimization. Attend bedside patient care during medical emergencies.
