Clinical Pharmacist - Home Infusion

The Clinical Pharmacist is responsible for coordinating all clinical agency activities associated with pharmaceutical services and products. Clinical Pharmacist will maintain competencies in sterile compounding and USP 797 requirements. Utilizes the EMR system for pharmacy review, record review and necessary clinical documentation. Provides clinical pharmaceutical consultation to physicians and staff, and serves as direct communicator with the prescribers when order changes are necessary or recommended.

Required qualifications: 

Graduation from an accredited school of pharmacy with didactic coursework and experiential rotations in pharmaceutical care Upon hire: Washington Pharmacist License

Preferred qualifications:

PharmD or Post-graduate degree in pharmacy, Or Completion of a post-graduate pharmacy residency in geriatrics Experience in clinical management, managed care and program development
