Major Responsibilities:

Participates in drug therapy management and documents the care provided in patients’ medical records.  Participates in interdisciplinary reviews of patients’ progress, as well as communicates pertinent patient information to other health care providers when patients change care settings to ensure continuity of care. Prepares medications by reviewing and interpreting physician orders; detecting therapeutic incompatibilities.Dispenses medications by compounding, packaging, and labeling pharmaceuticals.Contacts and confers with physicians and other practitioners to optimize drug therapy and advising of needed interventions.Assists patients in gaining access to medications (e.g., working with payers to resolve issues that may impede access to medication therapies) when applicable.Interprets and responds to clinical decision support and drug utilization review from internal and external systems on patients’ drug therapy. Provide individualized health promotion and disease prevention, including administration of immunizations where legal and authorized.Provides computer order entry, ensuring proper technique and accurate preparation of all pharmaceutical products.Supervises technicians as needed.Complies with state and federal drug laws as regulated by the state board of pharmacy, the drug enforcement administration, and the food and drug administration.Provides pharmacological information by answering requests of health care professionals.Counseling patients on drug therapies.Report all adverse drug reactions and drug related events to proper persons.Maintains all records need for the proper handling of 340b purchased medications and facilitates the most cost-effective means for the procurement of pharmaceuticals. Precepts Early Pharmacy Practice Experience (EPPE) rotation students, and Advanced Pharmacy Practice Experience (APPE) rotation students, and pharmacy residents consistent with academic credentials and clinical experience.Prepares presentations, publications and other information sharing activities for the health care community and general public.Participates in continuing education programs, meetings, training programs and related activities.

Required:

Doctor of Pharmacy (Pharm.D) degree or Bachelor of Science in Pharmacy (BSPharm) degree.Licensed in the Commonwealth of Massachusetts.   

Experience/Skills:
Required:

3 years ambulatory experience.  Must have essential skills to implement motivational interviewing skills to assist the patient in taking an active part in their health care and promote healthy lifestyles.    Must be able to cross-cover in a variety of practice areas within the pharmacy as requested by supervisor.
