Work Schedule: 

100% FTE. Day shift. Hours vary 6:00 AM – 6:00 PM. Up to every 4th weekend rotation required. Hours may vary based on the operational needs of the department. You will work at American Family Children's Hospital in Madison, WI.

We are seeking a Clinical Pharmacist – Pediatric Hematology and Oncology Inpatient / Outpatient to: 

Provide pediatric medication therapy management services as part of the interdisciplinary team to hematology and oncology patients to assure that drug therapy is safe, effective, efficient, and cost-effective. Provide training and education to Pharmacy Residents and PharmD students through practical and didactic experiences.  Participate in interdisciplinary patient care rounds, medication reconciliation, medication therapy monitoring, medication orders management, patient education, drug information, drug dosing, and various scholarly activities. 

The ideal candidate will have pediatric hematology/oncology experience. Preference given to candidates PGY2 with a focus in oncology and pediatrics or with significant oncology experience in pediatrics.
