JOB SUMMARY:
The incumbent of this position manages the clinical pharmacy section of the division, including developing, monitoring, evaluating and coordinating clinical pharmacy services that promote desirable patient outcomes. The incumbent will develop and implement policies, procedures, process guidelines, and protocols to ensure the provision of safe, effective and economical pharmacotherapy for all patients in the hospital, ensuring compliance with clinical practice standards including accreditation, applicable federal, state and local laws and regulatory standards. The incumbent sits on the Pharmacy and Therapeutic Committee and other key patient care/clinical committees. Responsibilities include direct supervision of clinical pharmacy staff who provide patient-focused pharmaceutical care. Other responsibilities include the development and implementation of quality improvement initiatives, fiscal controls, and management of human and capital resources, formulary management, and risk management functions for the clinical pharmacy program. Interprets physician prescriptions and medication orders. Acts as a drug information resource to patients, medical staff, nursing staff and ancillary department personnel. Compounds and dispenses prescribed medications and other pharmaceuticals for patient care by performing the related duties. Management of student opportunities within our facilities.

DUTIES AND RESPONSIBILITIES:
Supervises clinical pharmacy staff, performing the full range of supervisory functions, including: performance evaluations; staffing plans; training and development; assignment; disciplinary matters; team building; recruitment; etc. Assists the Assistant Directors in developing operational systems and procedures for the Pharmacy Department’s Clinical Division that are in accordance with the guidelines for the Accreditation of Healthcare Organizations, and state hospital codes. Assists the Director in coordinating the review, tracking and compilation of data regarding adverse drug events (i.e., medication incidents and adverse drug reactions). Issues statements on the clinical policies and procedures of the Pharmacy Department. Assists in the selection of drug products and the development of protocols. Develops communication systems to respond to problems or emergency requests from patient care units. Serves on a number of key clinical and patient safety committees. Attends conferences, seminars, and meetings as required. Participates in the generation of new knowledge relevant to the practice of pharmacotherapy, clinical pharmacy and medicine. Educates health care professionals, students, patients, and the public regarding rational drug therapy. Interprets and applies pharmacokinetic drug data, and recommends dosing regimens based on patient's medical history and disease state, ensuring optimal patient care. Provides patient education and counseling. Establishes, coordinates, implements and provides all aspects of clinical pharmacy activities with emphasis on therapeutic drug monitoring, clinical pathways, formulary management, pharmacoeconomics and patient outcomes through detailed reporting systems. Collaborates with physicians, participates in multidisciplinary team rounds and provides pharmacotherapy recommendations during the discussions Manages non-formulary requests for drugs, and participates in Pharmacy & Therapeutics Committee activities Performs initial assessments of drugs having pre-established criteria to be monitored, and monitors these drugs for follow-ups Uses computer applications or other automated systems such as spreadsheets, word processing, calendar, e-mail and database software in performing work assignments
BASIC PHARMACIST RESPONSIBILITIES:
Reviews prescriptions issued by physicians or other authorized prescribers to assure accuracy and determines formulas and ingredients needed. Compounds medications using standard formulas and processes such as weighing, measuring, and mixing ingredients. Monitors all posting onto the patient profiles in order that accuracy, drug allergies, drug interactions, duplicate therapy and incompatibilities may be realized and better patient care received. Checks patient medication orders prior to delivery to patient care areas to ascertain that the correct drug, dosage form, schedule and strength is being dispensed. Performs all aspects of patient care in an environment that optimizes patient safety and reduces the likelihood of medical/health care errors. Prepares IV admixtures and extemporaneous parenteral products under aseptic conditions. Directs Pharmacy Department personnel engaged in admixing, packaging and labeling pharmaceuticals. Answers questions and provides information to physicians, nurses, patients and other healthcare professionals on the appropriate use of medications, drug interactions, side effects, dosage, storage, etc. Ensures the maintenance and proper operation of equipment and supplies in the work areas. Maintains established procedures concerning quality control, security of controlled substances and disposal of hazardous waste. Enters data into the computer system (Pharmacy program) such as patient, prescribed medications and cost and maintains patient profiles and charge system. Instructs students and other medical personnel on matter pertaining to the Pharmacy Department. Reviews and monitors drug therapy that requires dosing adjustments as authorized by the physician. Provides pharmaceutical care to all ages of patients from newborns to the elderly. Understands that there are dosage variations between newborn, pediatric, adult and geriatric patients and knows where to find appropriate information. Treats patients and their families with respect and dignity. Identifies and addresses psychosocial, cultural, ethnic and religious/spiritual needs of patients and their families. Interviews and assists in the hiring process, trains personnel, plans, assigns and directs work, addresses complaints and resolves problems. Demonstrates ability to read, analyze and interpret pharmacy-related journals. Demonstrates ability to respond to common inquiries or complaints from customers, regulatory agencies or members of the hospital staff. Demonstrates ability to write articles for publication and ability to effectively present information to management, medical staff, hospital staff and public groups. Demonstrates ability to apply necessary advanced mathematical concepts such as exponents and logarithms. Demonstrates ability to apply mathematical operations to such tasks as dosage calculations, formulation of pharmaceutical preparations and data analysis. Demonstrates ability to define problems, collect data, establish facts and draw valid conclusions. Demonstrates ability to interpret an extensive variety of technical instructions and deal with several abstract variables. Completes annual education requirements. Maintains regulatory requirements. Maintains patient confidentiality at all times. Reports to work on time and as scheduled, and completes work within designated time. Completes in-services and returns in a timely fashion. Attends annual review and department in-services, as scheduled. Attends staff meetings in the absence of the Pharmacy Directory and reads and returns all monthly staff meeting minutes. Represents the organization in a positive and professional manner. Actively participates in performance improvement and continuous quality improvement (CQI) activities. Complies with all organizational policies regarding ethical business practices.
