Job Overview:
The Ambulatory Care Clinical Pharmacist Specialist in Endocrinology is responsible for ensuring safe, effective, and prompt medication therapy management for patients within the clinic setting. The specialist collaborates with endocrinologists to optimize patient care and outcomes and sees patients independently through collaborative practice agreements. They are aligned with target interdisciplinary programs and specialty services to deliver medication therapy management and ensure pharmaceutical care programs are appropriately integrated throughout the institution.
In this role, the Clinical Pharmacist Specialist participates in all necessary aspects of the medication-use system, providing comprehensive and individualized pharmaceutical care to patients. They serve as departmental resources and liaisons to other departments, hospital personnel, or external groups. The Clinical Specialist will also provide medication and practice-related education/training and actively serve as preceptors for Doctor of Pharmacy students and pharmacy residents.
Job Requirements:
Bachelor's Degree in Pharmacy; PharmD or Bachelor's of Science degree with equivalent decentralized hospital pharmacy experience (3 years) OR accredited PGY-1, PGY-2 residency Registered Pharmacist with Ohio Board of Pharmacy Licensure Clinical mastery of dedicated area of pharmacy practice consistent with the job posting 1-2 years of experience in Clinical Pharmacy practice Completion of ASHP-accredited PGY1 pharmacy residency or Board Certification in a related area of practiceJob Responsibilities:
Lead assigned patient service lines, clinical areas, and therapeutic programs, facilitating pharmaceutical care services, direct patient care programs, and medication utilization systems. Design and implement stewardship activities and restriction/surveillance programs. Track and evaluate assigned pharmacy programs for operational, quality, and financial efficiency, benchmarking against local and national best practices. Develop and oversee policies and procedures. Deliver direct patient care and clinical practice, including decentralized and service-based programs. Act as an active member of the multidisciplinary team and collaborate with healthcare providers within decentralized patient care areas to provide patient-centered care. Provide pharmaceutical services throughout the medical center, maintaining proficiency in hospital computer systems and medication ordering systems. Complete critical patient monitoring and review patient profiles/charts to identify, prevent, or mitigate drug-related problems and treatment failures. Participate in pharmacy operations and medication dispensing. Maintain proficiency in operational programs, central dispensing pharmacies, satellite dispensing pharmacies, and specialty pharmacy areas. Facilitate specialty medication procurement, ordering, and dispensing procedures. Facilitate experiential education and practice advancement. Maintain proficiency in preceptor roles and actively emulate education/mentorship skills. Develop student, resident, and staff training experiences/competencies and create new teaching and training opportunities. Communicate relevant information to other department members. Assume personal responsibility for professional development, particularly in ongoing clinical education.Other Job-Related Information:
