The critical care pharmacist specialist is responsible for providing expert pharmaceutical care to critically ill patients, collaborating with an interdisciplinary team and ensuring optimal medication management. This position will primarily cover medical ICU and will work with other critical care pharmacist specialists to ensure continuity of care for all critically ill patients. Responsibilities include patient assessment, medication management, managing formal pharmacokinetic, parenteral nutrition and anticoagulation consults, attending daily interdisciplinary rounds and documentation of patient care activities and interventions.
General Summary
Provides comprehensive pharmaceutical care to acutely ill patients in conjunction with a multidisciplinary team. Their advanced knowledge and practice experience facilitate the delivery of safe, effective, and appropriate medication therapy to their assigned patient populations. They have the expertise and advanced knowledge to be viewed as a system-level content expert in pharmacotherapy. Clinical Specialists actively participate in interdisciplinary committees, staff education, professional development, and performance improvement initiatives. In addition, they play a major role in the education of pharmacy students, PGY1 Pharmacy and PGY2 Critical Care pharmacy residents, medical residents, providers, nurses, and ancillary staff.
Essential Functions:
Provides medication therapy management services in collaboration with other members of the health care team including active participation in daily multidisciplinary patient care rounds.Provides consultative services pursuant to the department's applicable services which may include: pharmacokinetic evaluation, monitoring, and dosing, total parenteral nutrition (TPN), anticoagulation, renal dose adjustments, and intravenous to oral conversions.Assists with medication reconciliation during transitions of care.Provides educational services to patients and healthcare providers regarding medication therapy and/or medication safety concerns.Participates in the training and education of pharmacy students, PGY1 and PGY2 pharmacy residents, and physician residents. Participates in ongoing departmental preceptor development .Participates in quality, safety, and performance initiatives related to medication use at the organizational and departmental levels. Reviews and creates evidence-based protocols and order sets.Takes a lead role in collaboration on departmental process research activities, including mentoring of other staff through research initiatives.Adheres to WellSpan Health's medication-use policies and standard work. Develops and modifies polices and standard work related to the use of medications within the health system.Participates in the health system's formulary process, ensuring an appropriate formulary system is in place.Actively participates on department, facility, or health-system committees, including the Pharmacy and Therapeutics Committee, Formulary Committee and Drug Shortage Committee. Serves as pharmacy representative for area assigned committees and initiatives.Maintains professional growth and development through continuing education, workshops, and professional affiliations to keep abreast of latest trends in field of expertise.Provides outstanding service to all customers; fosters teamwork; and practices fiscal responsibility through improvement and innovation.Provides annual reporting to clinical leadership regarding initiatives and metrics for assigned area of responsibility.Common Expectations:
