Clinical Pharmacist - Transitions of Care at Providence Sacred Heart Medical Center - Spokane, WA

This position is a dedicated transitions of care pharmacist primarily focused on admission medication reconciliation for our hospitalized patients. Potential for leadership growth as this position oversees technicians and routinely precepts students and residents. A strong interest in transitions of care, teaching, quality improvement and leadership development is preferred.

Required qualifications:

PharmD Doctorate of Pharmacy Or Bachelor's Degree in Pharmacy      Upon hire: Washington Pharmacist License when eligible and where applicable, obtain a Board of Pharmacy Preceptor license Where applicable Basic Life Support-Provider (BLS)

Preferred qualifications:

Acute care hospital pharmacy experience, 1 year or equivalent experience
