PURPOSE OF POSITION
The purpose of the clinical pharmacist position is to review patient medication use on a specific unit or two, make recommendations for improved therapies, educate patients on their probable discharge medications and facilitate any assistance in acquiring or preparing medications for discharge. Additionally, the clinical pharmacist will provide medication education to nursing and physician personnel as required or desired. Response to medical emergencies is also a consideration of their duties.
RESPONSIBILITIES & EXPECTATIONS
Clinical Pharmacist Responsibilities:
Conducts comprehensive medication profile reviews to identify, prevent and resolve potential and actual drug related problems and to optimize drug therapy according to core measures, current guidelines and updates in the primary literature. Work with prescribers to achieve optimal outcomes for acute and post-discharge drug therapy issues.· Assist with medication reconciliation, patient education, facilitating compliance with core measures, prospective formulary management and appropriate documentation in the medical record
· Performs pharmacotherapeutic monitoring and provision of pharmaceutical care for patients in the area of service responsibility in a routine, continuous manner, taking into account the age and clinical condition of each patient
· Knowledgeable of drug/drug interactions, food/drug interaction, compatibility and stability of intravenous medications including chemotherapy agents and Total Parenteral Nutrition
· Prepares and/or supervises the preparation of extemporaneously compounded medications, including sterile products, in compliance with current USP 797 guidelines
· Precepts pharmacy residents and students as assigned by the pharmacy director or assistant manager.
· Assures the appropriate selection, dosing, administration, and dispensing of medication orders.
· Provides general and patient specific drug information to patients, physicians, nurses and, and other health care professionals to provide optimal pharmaceutical care to patients. Recommends formulary alternatives when appropriate.
· Responds to medical emergencies in area of service and facilitates medication administration as part of the code team.
· Documents education, monitoring and clinical activities in the medical record in an appropriate and succinct manner.
· Maintains control and security of medications at all times.
· Responsible for the oversight and supervision of activities of Pharmacy Technicians.
· Understands regulatory requirements and standards affecting his/her scope of practice.
· Follows medication use policies and complies with drug formulary policies and procedures, including enforcement of the Pharmacy and Therapeutics Committee approved therapeutic interchanges and non-formulary requests.
· Provides education to providers when they have problems issues entering medication orders into the Cerner Millennium information system.
· Enter medication variances in Midas & enters clinical interventions identified in Sentri7 and by self into Quantifi software (or other current intervention tracking software used by Aultman Pharmacy.
· Assists in the training and development of new pharmacy employees.
Performs other duties as assigned by the supervisor.Job Requirements
Education: Bachelor of Science in Pharmacy or Doctor of Pharmacy degree (PharmD) from a college or a university accredited by the American Association of Colleges of Pharmacy, Licensure/Certification/Registration: Pharmacy License (RPh) in the State of Ohio, Board Certification or eligibility within 1 year Experience: PGY-1 Residency in acute care setting. Minimum one year related experience preferred. Full understanding of Aultman Hospital pharmacy dispensing functions (up to 6 months assigned).
